摘  要:目的探讨不同抗精神病药物对部队精神分裂症患者神经行为功能的影响。方法将131例部队精神分裂症患者随机为氯氮平组、利培酮组和氯丙嗪组,在治疗前和治疗24周时分别对其进行WHO-NCTB、PANSS评定,并与42例正常对照进行对比。结果治疗后,三组的PANSS总分均显著下降(P<0.01);氯氮平组、利培酮组在治疗24周后的平均反应时、最快反应时、利手的手工敏捷度、正确打点数、数字译码与治疗前相比有显著性差异(P<0.05,P<0.01);氯丙嗪组治疗24周后的数字跨度和视觉记忆显著低于治疗前(P<0.05),其余无明显变化(P>0.05);治疗24周时,氯氮平组和利培酮组的多项NCTB(如数字跨度、视觉记忆、敏捷度、数字译码等)显著高于氯丙嗪组(P<0.05,P<0.01),与正常对照组无显著性差异(P>0.05);而氯丙嗪组的NCTB(反应时、数字跨度、视觉记忆、敏捷度、数字译码等)均与正常对照组有显著性差异(P<0.05,P<0.01)。结论利培酮和氯氮平能够较好地改善部队精神分裂症患者的神经行为功能,而氯丙嗪对此作用不明显,甚至有恶化的作用。Objective To explore the effects of different antipsychoties on neurobehavioral function in the military schizophrenic patients. Methods A total of 131 military schizophrenic patients were randomized into clozapine group, risperidone group and chlorpromazine group, and were assessed with WHO-NCTB and PANSS at baseline and at the 24th weekend of the treatment. Scores of WHO-NCTB and PANSS of patients were compared with those of 42 healthy controls. Results Total score of PANSS in the three groups decreased significantly after 24-week treatment ( P 〈 0.01 ). Compared with the baseline, there were significant differences in average reaction time, fastest reaction time, Santa Anna manual dexterity test,correct tapping dots and digit symbol test in both clozapine group and risperidone group after 24- week treatment ( P 〈 0.05 or P 〈 0.01 ). Scores of digit span and Benton visual retention test in chlorpromazine group decreased significantly when compared with those at baseline ( P 〈 0.05 ). At the 24th weekend of the treatment, many NCTBs such as digit span, visual retention, manual dexterity and digit symbol test in clozapine group and risperidone group were significantly higer than those in chlorpromazine group ( P 〈 0.05 or P 〈 0.01 ), but showed no significant differences when compared with those in control group ( P 〉 0.05 ). There were significant differences in many NCTBs such as reaction time, digit span, visual retention, manual dexterity and digit symbol test between chlorpromazine group and control group (P 〈 0.05 or P 〈 0.01 ). Conclusion Clozapine and risperidone could improve the neurobehavioral function of the military schizophrenic patients rather than chlorpromazine.
