奥曲肽预防伊立替康引起的迟发性腹泻的临床研究

摘　　要：目的:评估奥曲肽对伊立替康化学治疗引起迟发性腹泻的预防作用。方法:接受伊立替康化学治疗的晚期结直肠癌患者116例,按治疗时间分组,预防用药组(n=60)化学治疗前皮下注射奥曲肽0.1 mg,每8 h一次,治疗5 d;对照组(n=56)化学治疗前不接受奥曲肽治疗。观察迟发性腹泻的发生率,比较两组住院费用和住院时间,观察不良反应发生情况。结果:预防用药组与对照组相比,迟发性腹泻(16/60比32/56)、严重腹泻(6/60比16/56)、因腹泻导致化学治疗疗程改变(3/60对比11/56)的发生率均明显降低(P<0.05);住院费用明显降低(P<0.05);住院时间明显缩短(P<0.05)。预防用药组奥曲肽相关不良反应主要有注射部位疼痛、颜面潮红、头痛头晕,但均不影响化学治疗。结论:奥曲肽可以降低伊立替康引起的迟发性腹泻、严重腹泻的发生率,用药安全且可降低住院费用和缩短住院时间。Objective： To evaluate the preventive effect of octreotide on irinotecan-induced delayed-onset diar- rhea. Methods： Consecutive patients who were about to receive irinotecan based chemotherapy were recruited.One hundred and sixteen patients were divided into two groups based on the timing of treatment. In preventive medication group, subcutaneously injection of octreotide before chemotherapy was applied, while no octreotide was giv- en in control group. The incidence rate of delayed-onset diarrhea, cost of hospitalization, length of hospital stay and the possible side effects of preventive octreotide usage were determined in this study. Results： In treatment group, there were significantly lower incidence rates of delayed onset diarrhea （16/60 vs 32/56）, severe diarrhea （6/60 vs 16/56）, and alteration of chemotherapy courses due to diarrhea （3/60 vs 11/56）, compared with those in control group （P 〈 0. 05 ）. Preventive usage of octreotide also resulted in significant reduction of cost of hospitalization and hospital stay （P 〈 0. 05 ）. Side effects of octreotide injection, including mild to moderate pain at the injection site, redness of the face, headache and dizziness, required no treatment or discontinuation of chemotherapy. Conclusion： Preventive usage of octreotide is safe and effective in reduction of delayed-onset diarrhea, severe diarrhea, cost of hospitalization and length of hospital stay.
