彩色多普勒超声在涎腺良恶性肿块鉴别诊断中的价值

摘　　要：目的探讨彩色多普勒超声对涎腺良恶性肿块鉴别诊断的应用价值。方法回顾性分析43例涎腺肿块的声像图表现、内部和周边的血流分布及多普勒频谱,将诊断结果与病理结果对照分析。结果良性肿块25例,超声诊断符合21例,恶性肿块18例,超声诊断符合14例,符合率分别为84%、78%,统计学比较P>0.05。良性肿块中92%可见清晰包膜,恶性肿块无包膜或包膜不完整;良性肿块血流PSV仅16%>35cm/s,而恶性肿块血流PSV有61%>35cm/s,两者在二维声像图及血流峰值方面均有显著性差异。结论彩色多普勒超声在涎腺肿块良恶性的鉴别诊断中能提供有效的依据,具有重要的鉴别诊断价值。Objective To investigate the application of Color Doppler Ultrasound Diagnostic Method in the Diagnosis of Salivary Gland Masses.Methods The ultrasound images in 43 patients with the salivary gland mass,the blood flow distribution within and around the salivary gland mass and its Doppler Spectrum was analyzed,the result of CDU were compared with those of pathology.Results 21 out of 25（84 %） cases of benign masses,14 out of 18 cases of malignant masses accurate diagnosed by ultrasonic diagnosis（P0.05）.The infarcts can be seen clearly in 92% of benign masses,but there are no infarcts or not complete infarcts in malignant masses.16% of the PSV of blood flow is over 35cm/s in benign masses,but there are 61% are over 35cm/s in malignant masses.The difference in Two-dimensional ultrasonic features and peak flow velocity between them is significant.Conclusion Color Doppler Ultrasound Diagnostic Method can offer an effective resolving to the diagnosis of salivary gland mass,and it has important accessory diagnostic value..
