摘  要:目的 :研究神经心理学测验和脑SPECT对帕金森病(PD)的诊断价值。材料和方法 :对30例正常老人(NA)和31例PD进行神经心理学测验(MMSE ,FOM ,DST ,RVR ,BD)和SPECT检查 ,比较两组认知功能和脑血流灌注特点。结果 :两组间各测验(MMSE ,FOM ,DST ,RVR ,BD)结果均有显著差异 ,NA各测验得分均高于PD。与NA相比 ,PD双侧顶叶、颞叶、基底节、丘脑的放射性计数比值显著降低(P<0.05)。相关和回归分析提示PD患者脑SPECT放射性计数值比值与神经心理学测验结果有一定关系 ,FOM分数与顶叶、左颞叶、左丘脑分别有显著性相关(P<0.05) ,提示可用FOM间接反映上述区域脑血流灌注情况。结论 :神经心理学测验和脑SPECT可作为诊断PD的辅助手段。Purpose:To explore the characteristics and diagnotic values of neuropsychological tests and SPECT with 99mTc-HMPAO in Parkinson's disease (PD).Materials and Methods:Five neuropsychological tests and SPECT were performed in 30 normal adults(NA) and 31 PD to observe their cognitive functions and cerebral perfusion.Results:cognitive functions assessed by neuropsychological tests(MMSE,FOM,DST,RVR,BD)and cerebral perfusion examined by SPECT in PD group were significant different from that in NA.The ratioes of all cognitive functions tested were significantly lower in PD group Compared with that in NA group.The cerebral perfusion of PD was significantly decreased in bilateral frontal,parietal,thalamus cortex and basal ganglia in PD compared with those of NA.Relationship and regression analysis showed that there was certain relation between neuropsychological tests and cerebral perfusion.The perfusion in frontal,left parietal and left thalamus cortex was significantly correlated with FOM results(P<0.05),indicating that FOM could reflex indirectly the perfusion in these areas.Conclusion:Neuropsychological tests and brain SPECT can be used as auxiliary means in the diagnosis of PD.
