延续护理在乳腺癌化疗患者居家管理中的应用

摘　　要：目的探讨延续护理在乳腺癌化疗患者居家管理中的应用情况,为癌症化疗患者提供一种新的居家管理模式。方法采用奥马哈系统的延续护理评价系统,分别在乳腺癌第1个化疗疗程治疗结束后评价(t1),第1(t2)、第2(t3)、第3(t4)个化疗疗程居家休息的第2周评价。结果不同时间患者认知、行为、现状评分比较,差异均有统计学意义(P<0.05)。t2较t1、t3较t2、t4较t3认知、行为、现状评分均明显升高,差异有统计学意义(P<0.05)。结论延续护理从环境、心理社会、生理和健康行为4个领域全方位护理介入,既解决了乳腺癌化疗患者治疗间期居家管理中的问题,又为护理提供了标准化的、可遵循的居家自我管理康复方案。Objective To invesUgate the apphcatlon ol extendecl care m home management ot breast cancer paUents treated with chemotherapy in order to provide a home management model for cancer patients treated with chemotherapy. Methods Omaha extended care evaluation system was used after the first chemotherapy cycle and the second week at home after the first, second and third chemotherapy circle. Results Cognition, behavior and status quo scores of the patients in different time showed statistically significant differences （ P 〈 0.05 ） . Cognition, behavior and status quo scores in tz, t3 and t4 were significantly high- er than those of t1, t2 and t3 respectively （ P 〈 0. 05 ） . Conclusion Extended care intervenes nursing comprehensively in terms of environmental, psyehosocial, physical and behavioral areas. It can solve the home management problems between treatments for breast cancer patients treated by chemotherapy, and can provide standardized and structured program for nursing.
