摘 要:目的:探讨细支气管肺泡癌患者低氧诱导因子1α(HIF-1α)、环氧化酶2(COX2)的表达及临床意义。方法:采用免疫组化法检测60例细支气管肺泡癌蜡块标本中HIF-1α、COX2表达,采用Kaplan-Meier法分析肿块直径及HIF-1α、COX2表达与细支气管肺泡癌预后的关系。结果:细支气管肺泡癌肿块直径≥3 cm组中HIF-1α、COX2阳性表达率均明显高于直径<3 cm组(P<0.05);肿块直径≥3 cm组中COX2阳性表达率与淋巴结转移相关(P<0.05),肿块直径<3 cm组中COX2阳性表达率与淋巴结转移无关(P>0.05);HIF-1α与COX2表达呈正相关(r=0.366,P<0.05);5年累积生存率单因素分析显示,细支气管肺泡癌预后与肿块直径、COX2表达相关(P<0.05),与HIF-1α表达无关(P>0.05),多因素分析显示,肿块直径≥3 cm与COX2高表达是细支气管肺泡癌生存期的独立危险因素之一(P<0.05)。结论:细支气管肺泡癌肿块直径≥3 cm,HIF-1α、COX2水平显著上调,促进淋巴结转移,提示患者预后差。Objective To discuss expression and clinical significance of HIF-1α and COX2 in bronchioalveolar carcinoma.Methods Expression of HIF-1α and COX2 were detected by using immunohistochemical method in 60 cases of bronchioalveolar carcinoma,Kaplan-Meier method was used to analyze the relation between the tumor diameter and expression of HIF-1α and COX2 with prognosis of bronchioalveolar carcinoma.Results Positive rate of HIF-1α and COX2 in tumor diameter greater than 3 cm was obviously higher than diameter less than 3 cm(P 0.05);the rate of expression on COX2 was related with node metastasis(P 0.05)in tumor diameter greater than 3 cm group,the rate of expression on COX2 was not related with node metastasis(P 0.05)in tumor diameter less than 3 cm group.Expression of HIF-1α was correlated with COX2(r = 0.366,P 0.05).Five-year cumulative survival rate of single-factor analysis showed that prognosis of bronchioalveolar carcinoma was related with tumor diameter and expression of COX2 instead of HIF-1α,multivariate analysis showed that tumor diameter greater than 3 cm and expression of COX2 was independent risk factor of prognosis of bronchioalveolar carcinoma.Conclusion Diameter greater than 3 cm of bronchioalveolar carcinoma could significantly increase the level of HIF-1α and COX2,and promote lymph node metastasis,those patients have poor prognosis.
