神经心理测定和事件相关电位P300对躯体化障碍认知功能的评价作用研究

摘　　要：目的探讨神经心理测定和事件相关电位P 300对躯体化障碍认知功能的评价作用。方法对56例躯体化障碍患者和50名正常对照者进行神经心理和事件相关电位P 300测定,将所得结果进行分析。结果①躯体化障碍组与正常对照组相比,事件相关电位成分中N 2、P 3波潜伏期延长,N 2、P 3波幅降低,差异有统计学性意义(P<0.01);②躯体化障碍组在领悟、数字广度、词汇、编码、填图、图片排列因子方面都比正常对照组明显的差,差异有显著性(P<0.05或P<0.01),躯体化障碍组在W CST总测验次数、持续错误数明显多于正常对照组(P<0.05);③N 2潜伏期与领悟、编码、填图、W CST持续错误数均呈负相关(P<0.05),P 3潜伏期与领悟、编码、填图、W CST持续错误数有相关性(P<0.05),P 3波幅与领悟、编码、持续错误数有相关性(P<0.05或P<0.01)。结论事件相关电位P 300能较好地反映躯体化障碍的认知功能,可以作为躯体化障碍的1项客观电生理参考指标用于临床。Objective To explore the evaluation of neuro-psychological test and event-related potential P300 on cognitive function in patients with somatization disorder.Methods A total of 56 patients with somatization disorder and 50 normal controls were tested with WAIS,WCST and event-related potential P300.The results were analyzed with t-test and correlation analysis.Results ①Compared with the normal controls,the latency and amplitude of N2 and P3 of event-related potential of patients with somatization disorder prolonged and descended significantly（P0.01）.②The scores of comprehension,numeral scope,vocabulary,coding,completion of drawing and arranging pictures of WAIS in patients group were lower than those in control group,the numbers of total testing and persistent error of WCST in patients were more than those in control group significantly（P0.05 or P0.01）.③The latencies of N2 and P3 were negative correlation with comprehension,coding and completion of drawing of WAIS and the number of persistent error of WCST（P0.05）.the amplitude of P3 was correlation with comprehension,coding and persistent error number（P0.05 or P0.01）.Conclusion Event-related potential P300 can better reflect the cognitive function of patients with somatization disorder.It can be used to diagnose somatization disorder in clinic as an objective electrophysiological index.
