散发性胆囊癌hMLH1和hMSH2蛋白表达检测及其临床意义

摘  要:目的 研究散发性胆囊癌组织中错配修复基因human mutL homolog 1(hMLH1)和human mutS homolog 2(hMSH2)蛋白的表达,为揭示其与胆囊癌发生和转移的关系提供依据.方法 47例胆囊癌患者,采用Envision免疫组化法检测胆囊癌组织中hMLH1和hMSH2蛋白表达.结果 胆囊癌组织中hMLH1和hMSH2蛋白阳性例(率)分别为24例(51.06%)和20例(42.55%).肝脏浸润阴性组hMLH1蛋白的阳性率(19/29,65.51%)高于肝脏浸润阳性组(5/18,27.78%),差异有统计学意义(χ2=6.33,P<0.05);淋巴结转移阴性组hMLH1蛋白的阳性率(19/27,70.37%)高于淋巴结转移阳性组(5/20,25.00%),差异有统计学意义(χ2=9.46,P<0.01);Nevin分期Ⅰ+Ⅱ+Ⅲ期组hMLH1蛋白阳性率(17/23,73.91%)高于Nevin分期Ⅳ+Ⅴ期组(7/24,29.17%),差异有统计学意义(χ2=9.41,P<0.01).而hMSH2蛋白阳性率在各临床病理因素组间的差异均无统计学意义(均P >0.05).结论 hMLH1的缺失表达与胆囊癌浸润和转移有关,对胆囊癌的发生发展具有重要意义.Objective This study is to determinate the expressions of mismatch repair proteins human mutL homolog 1 (hMLH1) and human mutS homolog 2 (hMSH2) in patients with sporadic gallbladder carcinoma and investigate the relationships between hMLHI,hMSH2 and the tumor occurrence and metastasis. Methods 47 patients with sporadic gallbladder carcinoma from January 2003 to January 2006 were selected and the expressions of hMLH1 and hMSH2 proteins in carcinoma tissue were detected by Envision immunohistochemical techniques. Results The positive rates of hMLH1 and hMSH2 proteins in patients with sporadic gallbladder carcinoma were 51.06% (24/47) and 42.55% (20/47). The positive rate of hMLH1 in liver invasion negative group (19/29,65.51%) was significant higher than liver invasion positive group (5/18,27.78%)(X 2=6.33,P〈0.05);the positive rate of hMLH1 in lymph node metastasis negative group (19/27,70.37%) was significant higher than liver invasion positive group (5/20,25.00%)( X 2=9.46,P〈0.01); the positive rate of hMLH1 in nevin I+ II + III stages group (17/23,73.91%) was significant higher than IV+V stages group(7/24,29.17%)( X 2=9.41,P〈0.01). Conclusion The expression of hMLH1 protein is closely associated with progenesis and metastasis of sporadic gallbladder carcinoma and is of great value for ocurrence and improvement of sporadic gallbladder carcinoma.
