原发性闭角型青光眼晶体摘除前后前房深度和房角的对比

摘　　要：目的：了解原发性闭角型青光眼治疗中联合晶体手术的加深前房效果和临床意义。方法：５４眼原发性闭角型青光眼行白内障青光眼联合手术。其中ＥＣＣＥ＋ＩＯＬ＋小梁切除３２眼，ＥＥＣＥ＋ＩＯＬ＋虹膜周切１４眼，ＥＣＣＥ＋ＩＯＬ８眼。术前常规作Ａ超测前房深度和房角镜检查，术后三月后作同样检查。结果：术前平均前房深度２．１４±０．２３ｍｍ，术后平均前房深度４．３７±０．６７ｍｍ，经统计学分析有显著差异。术后比术前房角开放，也有显著差异，但房角粘连改变不明显。结论：提示在原发性闭角型青光眼治疗中，白内障手术可以加深前房，对开放房角和治疗有益。青光眼白内障联合手术有一定优点，掌握好指征可以取得良好效果。To evaluate the results and effect of glaucoma combined cataract operation that deeping the anterior chamber in treated primaly angle closure glaucoma.Method:54 eyes primaly angle closure glaucoma were operated.32 eyes were ECCE+IOL+trabeculectomy,14 eyes were ECCE+IOL+peripheral iridectomy,other 8 eyes ECCE +IOL were performed.All eyes have had examinations that A ultrasonography and gonioscopy in the times of before operations and after operations over three monthes.Results:The anterior chamber deep befor operations is 2 14±0 23 mm,and the after operations is 4 37±0 67mm.Had closed anterior chamber angle were opened,but their adhesion between iris and trabeculum have not obviously changes.Conclussion:To treat primary angle closure glaucoma,extracting the cataract can deepen the anterion chamber.it is beneficial.Glaucoma operafion combined cataract extraction have some advantages.
