摘  要:目的 探讨背景信号抑制磁共振弥散加权成像(diffusion weighted imaging with background signal suppression,DWIBS)检查对急性胰腺炎的诊断价值.方法 对81例急性胰腺炎患者及93例正常对照组进行常规MRI和DWIBS检查,测量DWIBS图像上胰腺及肝脏的信号强度,计算胰腺与肝脏的相对信号强度比(SIR).结果 DWIBS显示急性胰腺炎患者胰腺的信号强度明显高于肝脏(SIR=2.38),正常对照组的胰腺与肝脏信号相似(SIR=1.33),两者差异具有统计学意义(t=-17.889,P〈0.01).SIR值的ROC曲线分析显示,DWIBS序列的曲线下面积0.983,当诊断界值SIR〉 1.67时灵敏度为93.83%,特异度为91.4%,Youden指数0.85.预设的DWIBS专用窗位30,窗宽60有助于病变的显示.常规MRI检查的阳性率为88.9%,DWIBS检查的阳性率为97.5%(P〈0.05).结论 DWIBS结合常规MRI检查可明显提高急性胰腺炎的检出率.Objective To assess the diagnostic value of MR diffusion weighted imaging with background signal suppression (DWIBS) in acute pancreatitis. Methods 81 cases of acute pancreatitis and 93 cases of normal controls underwent routine MRI and DWlBS. The signal intensities of pancreas and liver were measured on I)WIBS. The signal intensity ratios(SIRs) of pancreas and liv- er were calculated. Results The signal intensity of pancreas in acute pancreatitis was higher than that of liver on I)WIBS(SIR 2.38}. In control group, the signal of pancreas and liver was similar( SIR=1.33)and there was statistically difference hetween them (t=17.889,P〈0.01J. In ROC analysis of SIR, area under thecurve(AUC) was0.983 onI)WIBS, when the cut off levelSlR-〉 1.67, the sensitivity, specificity and the Youden index were 93.83%,91.4% and 0.85, respectively. The special window level 30, window width 60 were useful in displaying the lesions. The positive ratios were 88.9% in routine MRI and 97.5% in DWIBS(P〈. 0.05). Conclusion DWIBS combined with routine MRI can improve the sensitivity and accuracy in the diagnosis of acute pancreatitis.
