摘　　要：目的探讨Survivin和PTEN蛋白在壶腹癌组织中的表达及其临床意义。方法用免疫组化EnVision法对40例壶腹癌组织进行Survivin和PTEN蛋白的检测，并对8例正常壶腹组织进行对照研究。结果壶腹癌组织中33例Survivin蛋白呈阳性表达，阳性率为82．5％，对照组中无阳性表达，两组阳性率比较差异有统计学意义（P〈0．01）；Survivin表达与十二指肠浸润、胰腺浸润、淋巴结转移均有明显相关性（P〈0．05）。PTEN蛋白在壶腹癌组中20例阳性表达，阳性率为50％，而对照组8例均为阳性表达，两组阳性率比较差异有统计学意义（P〈0．01）；PTEN表达与十二指肠浸润、胰腺浸润、淋巴结转移均有明显相关性（P〈0．05）；经Spearman等级相关性分析表明，Survivin与PTEN蛋白表达呈负相关（r=-0．57，P〈0．01）。结论Survivin和PTEN的异常表达与壶腹癌的发生发展有关，可作为壶腹癌预后的参考指标。Objective To study the expressions of survivin and PTEN in ampullary carcinoma and their clinical significance. Methods The expressions of Survivin and PTEN proteins were tested by EnVi- sion immunohistochemistry in 40 cases of ampullary carcinomas and 8 cases of normal ampulla of vater as control. Results The positive rate of Survivin in ampullary carcinomas was significantly higher than normal human controls （82. 5% vs 0% , P 〈0. 01 ）. The differences of the expression of Survivin in ampullary carcinoma of duodenal invasion, pancreatic invasion, and lymph node metastasis were significant （ P 〈 0.05 ）. Significant differences were also observed in the expression rate of PTEN between the patients with ampullary carcinoma and the normal controls （50% vs 100%, P 〈 0. 01 ）. The differences of the expression of PTEN in ampullary carcinoma of duodenal invasion, pancreatic invasion, and lymph node metastasis were significant （ P 〈 0.05 ）. Significantly negative correlation was found between the expression of Survivin and PTEN by using spearman correlation analysis （ r = - 0. 57, P 〈 0. 01 ）. Conclusions The abnormal expression of Survivin and PTEN gene may promote tumor genesis and progression of ampullary carcinomas and it may be used as the marker for prognosis.
