甲氧氯普胺治疗偏头痛疗效的Meta分析

摘　　要：目的探讨甲氧氯普胺治疗偏头痛的疗效。方法检索1970年01月～2011年06月MED-LINE、EMbase、ScienceDirect、Cochrane临床对照试验资料库(CCTR),以及国内的维普、CBM、CNKI数据库中关于甲氧氯普胺治疗偏头痛的随机对照研究的文献,按纳入、排除标准选择文献,并采用RevMan4.2软件进行Meta分析。结果共纳入16项随机对照研究,其中甲氧氯普胺治疗者985例,安慰剂对照者267例,其他药物治疗者816例。Meta分析显示,甲氧氯普胺治疗偏头痛的缓解率明显高于安慰剂治疗(OR=3.02,95%CI:2.12～4.30,P<0.001);但与其他药物治疗的相比差异无统计学意义(OR=1.14,95%CI:0.72～1.82,P=0.57);单用甲氧氯普胺治疗偏头痛的缓解率与使用其他药物治疗相比差异无统计学意义(OR=0.93,95%CI:0.34～2.53,P=0.89)。甲氧氯普胺治疗偏头痛的不良反应发生率明显低于其他药物治疗(OR=0.58,95%CI:0.44～0.77,P<0.001)。结论采用甲氧氯普胺治疗偏头痛的疗效确切,且不良反应相对较少,可作为治疗偏头痛的备选药物。Objective To evaluate the curative effect of Metoclopramide on treating migraine.Methods The randomized controlled trials of Metoclopramide on treating migraine published during Jan 1970 to Jun 2011 on MEDLINE,EMbase,ScienceDirect,Cochrane Database of Controlled Trials,and VIP database,CBM,CNKI were searched,then the data were picked up by inclusion and exclusion criteria and was made meta-analysis by RevMan4.2 software.Results There were 16 randomized controlled trials coincided with the inclusion and exclusion criteria,including 985 cases of Metoclopramide treatment,267 cases of placebo control,816 cases of other drugs treatment.Meta-analysis showed that the remission rate of Metoclopramide treating was significantly higher than the placebo on migraine(OR=3.02,95%CI: 2.12-4.30,P<0.001);but there was no statistical significant of the remission rate between Metoclopramide treating and the other drugs(OR=1.14,95%CI: 0.72-1.82,P=0.57);and there was no statistical significant of the remission rate between only Metoclopramide treating and the other drugs(OR=0.93,95%CI: 0.34-2.53,P=0.89).The incidence of adverse reaction of Metoclopramide treating was significantly lower than the other drugs on migraine(OR=0.58,95%CI: 0.44-0.77,P<0.001).Conclusions Metoclopramide is effective on treating migraine with little adverse reaction,which can be used as an alternative drug to treat migraine.
