摘　　要：目的 探讨CD15抗原的表达及含量与胆囊癌的临床病理意义。 方法 应用微波-LSAB免疫组织化学法和图象分析技术,检测45例胆囊癌,17例胆囊腺瘤和10例慢性胆囊炎组织中CD15抗原表达及含量。 结果 CD15抗原表达阳性率及阳性细胞积分A值,在胆囊癌中(71％,4.0±3.2)显著高于胆囊腺瘤(35％,0.9±0.3)和慢性胆囊炎(20％,0.4±0.1)(P<0.05,P<0.01);胆囊癌Ⅱ,Ⅲ级(90％,5.4±1.7)显著高于Ⅰ级(58％,2.2±3.1)(P<0.05,P<0.01);有转移的肿瘤(88％,6.5±3.5)(P<0.05,P<0.01);显著高于无转移的肿瘤(52％,3.3±2.1)(P<0.01)。 结论 CD15抗原表达与胆囊癌的发生、细胞增殖、分化和转移相关,对判断胆囊癌的恶性程度、预测生物学行为和预后具有一定临床意义。AIM To study the clinicopathological significance of CD15 antigen expression and its contents in human carcinoma.METHODS CD15 expression and its contents in 45 cases of carcinoma of the gallbladder, 17 cases of adenoma of the gallbladder and 10 cases of chronic cholecystitis were evaluated by microwave-LSAB immunohistochemical technique combined with image analysis. RESULTS Positive rate of CD15 and its integral absorbance (optic density) in carcinoma of the gallbladder were 71% (32/45) and 4.0±3.2 respectively, significantly higher than 35% (6/17) and 0.9±0.3 in adenoma of the gallbladder, 20% (2/10) and 0.4±0.1 in chronic cholecystitis (P<0.05, P<0.01). In grade Ⅱ-Ⅲ cancer, they were 90% (17/19) and 5.4±1.7, significantly higher than 58% (15/26) and 2.Ⅱ3.1 in grade Ⅰ cancer (P<0.05, P<0.01). In lymph node positive cases, they were 88% (21/24) and 6.5±3.5, significantly higher than 52% (11/21) and 3.Ⅲ2.1 in negative ones (P<0.025, P<0.01). CONCLUSION CD15 expression and its contents could be involved in aggresiveness of carcinoma of the gallbladder and might be a useful indicator to evaluate the malignancy and biological features, and could be considered as a good prognostic predictor for patients with carcinoma of the gallbladder.
