保留乳头乳晕复合体的乳腺癌改良根治术：Ⅰ期背阔肌脂肪瓣乳房重建

摘　　要：目的探讨保留乳头乳晕复合体（nipple—areolarcomplex，NAC）的乳腺癌改良根治术——I期背阔肌脂肪瓣重建乳房的方法及临床应用。方法自2006年1月至2008年12月，对16例早期乳腺癌行保留NAC的改良根治术，同时取部分或全部背阔肌脂肪瓣行I期乳房重建。结果16例手术均获得成功，无背阔肌脂肪瓣坏死，无乳房皮瓣及背部皮肤坏死。重建乳房美观，效果为优、良15例（93．8％），一般1例（6．2％）；中位随访时间为44个月（36—60个月），均无瘤生存。结论保留NAC的乳腺癌改良根治术——Ⅰ期背阔肌脂肪瓣乳房重建术，可满足肿瘤根治和形体美容两方面要求，提高患者生存质量。Objective To discuss the benefits of nipple-areola complex（NAC）preseving radical modified mastectomy and immediate reconstruction with latissimus dorsi fat flap for early breast cancer. Methods From Jan 2006 to Dec 2008, 16 patients underwent NAC preseving modified radical mastectomy and immediate breast recon- struction with latissimus dorsi fat flap. Results All patients recovered smoothly. No fat flap or back skin necrosis happened. The reconstructed breasts had good apperance with 93. 8% satisfaction rate. During the follow-up of 44 months （ranging from 36 to 60 months）, all patients were disease-free. Conclusions The operation of NAC pre- seving modified radical mastectomy and innnediate reconstruction with latissimus dorsi fat flap is safe and effective. It can improve the life quality of patients by satisfing the needs of both beauty and tumor resection.
