摘  要:探讨细胞动力学在子宫内膜癌发病机制中的作用。方法 :对13例子宫内膜癌、癌周内膜 ,15例简单型增生过长内膜 ,13例复杂型增生过长内膜石蜡组织切片 ,作HE染色及原位末端标记 (TUNEL) ,在光镜下 ,对切片中的有丝分裂细胞及凋亡细胞进行计数。结果 :复杂型增生过长内膜有丝分裂指数 (MI)及细胞凋亡指数 (AI)均较简单型增生过长内膜显著增高 (P均 <0 0 1) ,但MI/AI比值两者无显著差异 ;子宫内膜癌灶MI、AI、MI/AI比值均较癌周内膜及复杂型增生过长内膜显著增高 (P <0 0 1,P <0 0 5,P <0 0 5)。结论 :在子宫内膜癌变的早期 ,细胞动力学遵循既定细胞数调节机制 ,即加速的细胞增殖通过加快的细胞凋亡予以平衡。当过度增殖的细胞不能通过相应增强的凋亡途径加以清除时 ,会导致DNA损伤和变异的积累 ,最终引起子宫内膜癌。Objective:To study the role of cell kinetics in the mechanism of endometrial carcinoma.Methods:Samples were obtained from 13 cases of endometrial carcinoma foci and tissues around them,13 cases of complex hyperplasia endometrium and 15 cases of simple hyperplasia endometrium.Routine HE dying and TdT-mediated dUTP nick end labeling(TUNEL)staining were used to identify mitosis and apoptosis.Light microscopy was used to quantify their percentages,mitotic index (MI)and apoptotic index (AI).Results:Both of MI and AI in the complex hyperplasia endometrium were significantly higher than those of simple hyperplasia endometrium(P<0 01,P<0 01),but there was no significant difference between the ratio of MI/AI.The MI,AI and the ratio of MI/AI in the endometrial carcinoma foci were all significantly higher than those of tissues around them as well as complex hyperplasia endometrium (P<0.01,P<0.05,P<0.05).Conclusions:The data showed the putative cell population regulatory mechanism was played for balancing increased apoptosis and mitosis at the early stage of the process of endometrial carcinogenesis.When the over-increased cells cann't be deleted through corresponding increased cell apoptosis,the accumulating of DNA damage and mutation will occures,which will finaly result in endometrial carcinoma.
