食管粘膜下鳞癌生物学特性及P^(53)蛋白表达的意义

摘　　要：目的　研究食管粘膜下鳞癌的生物学特性。方法　通过随访及SP免疫组化技术P53 蛋白在 3 0例食管粘膜下鳞癌的表达 ,研究其生物学特性。结果　 3 0例食管粘膜下鳞癌的P53 蛋白阳性表达率为 66.7% ,经病理证实术时淋巴结的转移率为 60 % ,术后各种转移率为 47.8% ,其中伴肺转移 4例 ,单纯肺转移 1例 ,其 1、2、3、4年生存率分别为 10 0 %、70 .0 %、3 1.3 %和 11.1%。结论　食管粘膜下鳞癌术时及术后转移率较高 ,术后肺转移率较高 ,P53 蛋白阳性表达率高 ,予后差。鳞化伴异型增生的食管腺上皮细胞的阳性表达提示食管粘膜下鳞癌可能发生于食管腺。? Objective To study the biological features of esophageal submucosal squamous cell carcinoma.Methods Analysis of the clinicaopathological features and of P 53 protein expression in 30 submucosal esophageal squamous cell carcinoma by SP immunohistochemical technique were performed,and the patients had been followed up for 1～4 years.Results The results showed that the total postive expression rate of P 53 protein was 66.7%,the metastasis rate of lymph nodes proved by pathological examination was 60% when the operation was done,and the postoperative metastasis rate was 47.8%,of which 4 had metastasized to lung,1 had purely metastasied to lung,and the survival rate were 100%,70.0%,31.3% and 11.1% for 1,2,3 and 4 years respectively.Conclusions The metastasis rate of submucosal esophageal squamous cell carcinoma was high while operation and postoperation,the positive expression rate of P 53 protein was high,the prognosis was poor.The positive expression of epithelial cells of esophageal glands with metaplasia and displasia showed that the submucosal squamous cell carcinoma may originate from the esophagus gland. 〔
