白血病化疗患者外周血血清铁蛋白、转铁蛋白以及转铁蛋白受体与临床疗效的关系

摘　　要：目的探讨白血病化疗患者外周血血清铁蛋白、转铁蛋白、转铁蛋白受体等铁代谢相关指标与化疗的临床疗效的关系。方法我科收治的白血病化疗患者36例,所有患者分别在治疗前、治疗后4周抽取外周静脉血采用化学发光法以及散射比浊法检测血清铁蛋白、转铁蛋白、转铁蛋白受体表达水平。收集患者临床资料与铁代谢指标进行相关性分析。结果治疗开始前,患者血清铁蛋白、转铁蛋白以及转铁蛋白受体分别为519.71±157.38ng·mL-1,3.63±0.51g·L-1,2.87±0.76mg·L-1,均明显高于正常水平,有统计学意义差异(P<0.05)。复发与未缓解患者三项指标的均值明显高于初发组以及缓解组(P<0.05)。结束后4周检测铁代谢指标,血清铁蛋白、转铁蛋白、转铁蛋白受体均值均显著降低,差异有统计学意义(P<0.05)。其中,血清铁蛋白水平仍高于正常均值(P<0.05),转铁蛋白与转铁蛋白受体均值低于正常均值(P<0.05)。完全缓解与部分缓解组铁代谢指标显著低于未缓解组,差异有显著性(P<0.05)。结论铁代谢指标与白血病患者化疗的临床疗效间有着正相关的联系。Objective To explore the clinical effect of detection of serum ferritin, sennn transferrifin, serum transferritin receptor in patients with leukemia. Methods 36 patients with leukemia were enrolled in the study. The serum ferritin, serum transferritin, serum transferrifin receptor were evaluated before and after the chemotherapy via chemiluminescence and nephelometry assay. The relationship between the in- dex and clinical effect were studied. Results The iron metabolism indexes including serum ferritin, serum transferritin, serum transferritin receptor were 519.71±157.38 ng·mL^-1, 3.63± 0.51 g·L^-1, 2.87±0.76mg·L^-1 respectively, which were significantly higher than control （P〈0.05）. The indexes of relapse and non- remission patients were significant higher than those of remission patients （P〈0.05）. After the chemothera- py, The serum ferritin, serum transferritin, serum transferritin receptor were reduced significantly （P〈0.05）. The index of complete remission and partial remission patients were significantly lower than those of the non-remission patients （P〈0.05）. Conclusion The index of iron metabolism was positively related with the clinical effect of chemotherapy for the patients with leukemia.
