紫杉醇对鼻咽癌患者血清中Flt-4水平及淋巴结转移情况的影响

摘　　要：目的观察紫杉醇对鼻咽癌患者血清中Flt-4表达及淋巴结转移的影响。方法选取我院收治的鼻咽癌患者并随机分成对照组25例,治疗组22例。对照组采用X线照射放疗,以面颈联合野+双耳前野+颈部双锁上区前切线野为主,实验组采用紫杉醇化疗。治疗2疗程后检测白细胞计数、淋巴结转移及血清中Flt-4水平,观察其毒副反应并随访观察其肿瘤复发情况。结果经紫杉醇治疗后,患者血清中Flt-4水平明显高于放疗患者(P<0.05),患者发生血细胞减少、恶心呕吐、脱发、腹泻、便秘等副反应较放疗轻,紫杉醇治疗后毒性反应的发生率为19.2%,显著低于放疗患者(P<0.05)。患者经紫杉醇化疗后其3年和5年内原发灶和颈部淋巴结转移灶的复发率均显著低于化疗组(P<0.05)。结论紫杉醇治疗晚期鼻咽癌,其3年及5年原发灶和远处转移的复发率均低于放疗,且对患者的毒副作用小,其作用可能与降低血清Flt-4的水平有关。Objective To observe the effect of litaxel on the level of Flt-4 and lymph node metastasis of patients with nasopharyngeal carcinoma.Method 47 patients with nasopharyngeal carcinoma patients admitted in our hospital were divided randomly into the experimental group（25 patients）and control group（22 patients）.Patients in the control group were given radiotherapy with X-ray irradiation,the use of face and neck before the joint field ＋ field ＋ ears and neck area before the double-locked the main wild tangent.Patients in the experimental group were given paclitaxel.The white blood cell count,lymph node metastasis and the level of Flt-4 in serum were detected after 2 course of treatment,the adverse effects were observed and the recurrence of primary focus and metastasis of lymph node of neck were followed-up.Results The level of Flt-4 in the experimental group was significantly higher than the control group（P0.05）,the adverse effects such as leucocytopenia,nausea and vomiting,alopecia,diarrhea,constipation and so on hanppend in the experimental group were lighter than that in the control group,the incidence of toxic reaction in the experimental group was 19.2% that was notablely lower than the control grouo（P0.05）.The relapse rate of primary focus and metastasis of lymph node of neck in 3-year and 5-year were both lower than the control group（P0.05）.Conclusion When patients with nasopharyngeal carcinoma at the advanced stage were treated with paclitaxel,the relapse rate of primary focus and metastasis of lymph node of neck in 3-year and 5-year of thee were both lower than treated with radiotherapy,and little toxic side effects happened,these effect may be associated with the decrease of the level of Flt-4 in the serum.
