卵巢恶性生殖细胞肿瘤的超声声像图分析

摘　　要：目的回顾15例卵巢恶性生殖细胞肿瘤的超声声像图以增强对其认识,提示超声在其诊断中的价值和临床意义。方法 2003年7月～2011年1月在我院经腹及阴道超声检查发现盆腹腔囊实性包块的患者经术后随访病理证实,并对其声像图回顾性分析。结果 534例盆腹腔囊实性包块患者中15例为卵巢恶性生殖细胞肿瘤,其中2例卵巢卵黄囊瘤(内胚窦瘤);2例未成熟畸胎瘤;11例混合性生殖细胞肿瘤。结论超声检查对卵巢恶性生殖细胞肿瘤的敏感性较高,但特异性较低,如能结合血清肿瘤标记物CA125、AFP、LDH、HCG,可提高诊断的准确率。Objective To review fifteen ultrasonic imaging cases in order to enhance their understanding and to awaken the ultrasonic＇s value and clinical significance. Methods Do ultrasonography retrospective analysis to the patients who were found huge solid and cystic mass existed in abdominal from July 2003 to January 2011. Results Among five hundred thirty four cases, fifteen cases were malignant ovarian germ cell tumors （MOGCT）, including two cases of yolk sac sarcomas, two cases of immature deformity embryo tumors and eleven cases of mixed germ cell tumors. Conclusion The ultrasonic exami- nation is highly sensitive to the malignant germ cell tumor at ovary, but the specificity is relatively lower. The accuracy can be increased if combining with the tumor markers CA125, AFP, LDH and HCG.
