摘  要:目的 比较谷胱苷肽 S转移酶 (GST π)、多药耐药基因 (MDR1)、多药耐药相关蛋白基因(MRP)、肺耐药相关蛋白基因 (LRP)在卵巢癌等肿瘤组织中的表达水平及与化疗疗效的关系 ,并探讨血浆GST π水平用于卵巢癌化疗监测的可能性。方法 用逆转录 聚合酶链反应 (RT PCR)检测GST π ,MDR1,MRP ,LRP在 2 70份恶性肿瘤组织中的表达 ,以放射免疫技术检测卵巢癌血浆中GST π的浓度。结果 在检测的肿瘤组织中 ,食管癌GST π阳性率为 80 0 % ,乳腺癌MDR1阳性率为 94 .7% ,而卵巢癌的LRP阳性率为 87.8%。在乳腺癌和卵巢癌中 ,以 2个以上的耐药相关标志共表达为主 (P <0 .0 5 )。联合检测几个基因 (MRP、GST π、MDR1、LRP) ,可以提高对化疗疗效的判断 (P <0 .0 1)。GST π在卵巢癌化疗前后水平均下降 ,而LRP基因表达水平术前化疗组显著高于术前未化疗组。结论GST π ,MDR1,MRP ,LRP在许多肿瘤中表达 ,对肿瘤耐药有直接作用。在卵巢癌和乳腺癌中 ,耐药相关标志以共表达为主 ,联合检测可以协助临床判断卵巢癌的化疗效果。LRP基因水平的变化和GST π血浆水平的变化对疗效判断也有一定价值。Objective To detect the expression of glutathione S transferase π(GST π),multidrug resistance gene(MDR1),multidrug resistance associated protein (MRP) and lung resistance protein (LRP) in clinical carcinoma (include breast carcinoma, lung carcinoma, esophageal carcinoma and ovarian carcinoma) by RT PCR.Methods The gene expression of GST π, MDR1, MRP, and LRP was determined by RT PCR. The plasma level of GST π was determined by radioimmunoassay.Results The expression rate of GST π, MDR1, MRP, and LRP was over 30% in detectable carcinomas. The highest expression rate was found in different carcinomas: 80% for GST π in esophageal carcinoma, 94.7% for MDR1 in breast carcinoma, and 87.8% for LRP in ovarian carcinoma. The coexpression rate of drug resistance associated marker was higher than that of expression ( P <0.05). The non responder to platium based combination chemotherapy showed a higher rate of MRP, GST π and (or) MDR1, LRP coexpression than the responder ( P <0.01) did. The plasma level of GST π was higher after chemotherapy than before chemotherapy.Conclusions GST π MDR1, MRP, and LRP are major factors associated with drug resistance. Coexpression is one of major featues of drug resistance associated markers expression. Combined drug resistance associated markers may help judge the changes after chemotherapy.
