摘  要:目的:探讨CyclinD1 和增殖细胞核抗原(PCNA)在食管鳞癌,癌旁正常上皮和非典型增生上皮的表达特点,并分析了CyclinD1 在食管鳞癌发生,发展中的作用及与临床病理特点的关系。方法:应用免疫组织化学S—P法对82 例食管鳞癌,33 例非典型增生上皮和23 例正常粘膜上皮进行了CyclinD1 和PCNA检测。结果:82 例食管鳞癌中57 例阳性表达,阳性率为69.5% ,非典型增生组阳性率为36.4% (12/33),正常粘膜上皮阳性率为8.7% (2/23),PCNA的平均阳性率在三组分别为75.6% ±13.5% ,36.6% ±9.8% ,4.5% ±0.5% ,食管鳞癌中CyclinD1 和PCNA 表达有显著的正相关(P< 0.05),CyclinD1 和PCNA的表达与组织学分级有关(P< 0.05),而与其它临床病理特征无关。结论:①食管癌组CyclinD1 阳性率显著高于非癌组,且非典型增生组亦明显高于正常组(P< 0.05),提示CyclinD1 在食管鳞癌的发生,发展中起重要作用。②CyclinD1 可作为提示食管鳞癌预后的参考指标之一。Objective:To study the expression of CyclinD1 and proliferating cell nuclear antigen(PCNA)in esophageal carcinoma and its correlation with clinic pathologic features.Method:Immunohistoche mical assay was performed with a S P kit to detect the expression of Cyclind1 and PCNA in 82 cases of esophageal carcinoma,23 cases of normal epithelium and 33 cases of atypical hyperplasia. Results:①57 cases of esophageal carcinoma ,12 cases of atypical hyperplasia and 2 cases of normal epithelium express CyclinD1,their positive rates are 69 5%,36 4%and 8 7% respectively ②all cases of esophageal carcinoma express PCNA,its mean positive rate is 67 4%③The statistically significant association is found between the expression of CyclinD1& PCNA and histological grade ④the statistically significant association is found between the expression of CyclinD1 and the expression of PCNA Conclusion:①CyclinD1 may play an important role in esophageal carcinoma genesis ②Combination of CyclinD1 and PCNA expression seems a reliable parameter for judging malignant degree of esophageal carcinoma.
