腹腔镜胆囊切除术在老年人胆囊疾病中的应用

摘　　要：目的探讨应用腹腔镜技术治疗老年患者胆囊疾病安全性及有效性。方法回顾性分析360例老年患者行腹腔镜胆囊切除术的临床资料。结果 360例中,慢性结石性胆囊炎247例,急性结石性胆囊炎68例,胆囊息肉32例,非结石性胆囊炎13例。中转开腹3例(0.83%),其余顺利完成腹腔镜下胆囊切除术(LC),无胆管损伤,无严重并发症,无手术死亡。住院3～12 d,平均7.5 d。结论只要经充分术前准备,掌握手术适应症,把握手术时机及技巧,做好围手术期处理,老年人行腹腔镜胆囊切除术是安全可行的手术方法。Objective To explore the feasibility and safety of laparoscopic cholecystectomy in the aged patients with benign lesions of the gallbladder.Methods The clinical data of laparoscopic chdecystectomy in 360 aged patients were analysed retrospectively.Results Among the 360 cases,there were chronic lithogenous cholecystitis in 247 cases,acute lithogenous cholecystitis in 68 cases,polyps in 32 cases,and acalculus cholecystitis in 13 cases.The 3 cases were converted to the open cholecystectomy（0.83%）.Other 357 patients had Laparoscopic Cholecystectomy（LC）successfully without bile duct injury,serious complication,and operative death.The 3~12 days were cost in hospital,averaged 7.5 days.Conclusion Under a carefully prepare before operation,Strict indication and command operative time and techrical skill,laparoscopic cholecystectomy is a safe and feasible operation for aged patients.
