CEA蛋白、CEA mRNA在大肠癌诊断中的应用价值

摘　　要：目的探讨癌胚抗原(CEA)蛋白、CEA mRNA的检测在大肠癌诊断中的价值。方法分别采用电化学发光免疫法和逆转录聚合酶链反应法检测42例大肠癌患者和45例健康对照者外周血CEA蛋白、CEA mRNA水平。结果大肠癌组CEA和CEA mRNA阳性率均明显高于正常对照组(P﹤0.05)。并且,Ⅲ～Ⅳ期大肠癌CEA和CEA mRNA阳性率明显高于Ⅰ～Ⅱ期(P﹤0.05)。CEA阳性率与大肠癌患者肿瘤大小和淋巴结转移无明显相关性(P﹥0.05)。CEA mRNA阳性率与大肠癌患者淋巴结转移明显相关(P﹤0.05),而与肿瘤大小无明显相关性(P﹥0.05)。结论血CEA蛋白、CEA mRNA在大肠癌患者中表达明显升高,其中CEA mRNA对于大肠癌的检测更为敏感,并且其表达与淋巴结转移密切相关,具有较好的临床诊断价值。OBJECTIVE To investigate the carcinoembryonic antigen（CEA）,the value of the protein,CEA mRNA detection in the diagnosis of colorectal cancer.METHODS The electrochemiluminescence immunoassay and reverse transcriptase polymerase chain reaction method were used to detect CEA protein,CEA mRNA levels of 42 cases of colorectal cancer patients and 45 healthy controls.RESULTS The CEA and CEA mRNA in colorectal cancer group were significantly higher than the normal control group（P﹤0.05）.And CEA and CEA mRNA-positive rate in III-IV colorectal cancer were significantly higher than that on stage Ⅰ-Ⅱ（P﹤0.05）.CEA positive rate of colorectal cancer patients had no significant correlation with tumor size and lymph node metastasis（P﹥0.05）.CEA mRNA-positive rate was significantly associated with lymph node metastasis of colorectal cancer patients（P﹤0.05）,while no significant correlation with tumor size（P﹥0.05）.CONCLUSION Blood CEA protein,CEA mRNA expression in colorectal cancer patients are significantly higher.CEA mRNA is more sensitive to the detection of colorectal cancer and is related with its expression and lymph node metastasis,with a better clinical diagnostic value.
