中脑周围非动脉瘤性蛛网膜下腔出血16例分析

摘　　要：目的探讨中脑周围非动脉瘤性蛛网膜下腔出血(perimesencephalic nonaneurysmal subarachnoid hemorrhage,PNSH)的临床特点,治疗经验和预后。方法回顾性分析16例中脑周围非动脉瘤性蛛网膜下腔出血病人的临床表现、影像学资料及治疗,随访预后情况。结果本组全脑血管造影均无阳性发现,病人临床症状轻,均无意识障碍及局灶性神经功能障碍,Hunt-Hess分级均为Ⅰ～Ⅱ级。随访6～72个月,平均46个月,未发生再出血,无永久性神经功能障碍,均基本恢复正常工作生活。结论中脑周围非动脉瘤性蛛网膜下腔出血临床症状较经典蛛网膜下腔出血(SAH)轻,并发症少,预后良好。OBJECTIVE To investigate the clinical features,treatment and prognosis of perimesencephalic nonaneurysmal subarachnoid hemorrhage（PNSH）.METHODS Retrospective analysis was used to investigate 16 cases of perimesencephalic nonaneurysmal subarachnoid hemorrhage.Clinical manifestation,imaging information,follow-up treatment,and prognosis were analyzed.RESULTS The 16 cases showed no positive imaging information,and had mild clinical symptoms.There were no disturbance of consciousness and focal neurologic dysfunction,with Hunt-Hess grading I-II.Followed up for 6-72 months,mean 46 months,the cases had no bleeding,no permanent neurologic dysfunction,and returned to normal life.CONCLUSION Clinical symptoms Of perimesencephalic nonaneurysmal subarachnoid hemorrhage is less than that of the classic subarachnoid hemorrhage（SAH）,and the prognosis is good with less complications.
