摘　　要：目的探讨原发性胃恶性淋巴瘤的临床特点 ,提高其诊断及治疗水平 ,提高患者生存率。方法回顾分析了100例胃原发恶性淋巴瘤的临床及随访资料与胃癌作比较 ,探讨其临床特点。结果胃原发恶性淋巴瘤与胃癌相比 ,发病年龄较轻 ,临床症状与体征不平行 ,往往呈多中心、多形性改变 ,术中病态貌似凶险 ,但切除率高。本病对化疗、放疗均较敏感 ,疗效优于其他肿瘤。结论胃原发恶性淋巴瘤由于其发病率较低 ,加之其独特的临床特殊性 ,临床工作者对其认识不足 ,易造成漏诊、误诊。对该病进行手术时 ,应持比胃癌更积极的态度进行手术 ,术后的化疗。Objective The clinical characteristics of primary gastric malignant lymphoma was further analysed in an effort to improved its diagnostic and therapeutic levels as well as patient’s survival .Methods The in-clinic and follow-up data of 100 patients with the malignancy were retrospectively reviewed and contrasted with that of those suffering from gastric cancer .Results In comparison with cancer , malignant lymphoma occurred frequently in younger individuals with symptoms lighter than signs; the endoscopic morphology looked more complicated ( often multicentric and polymorphic ) ; the operative resectability remained higher despite the enormous size with better prognosis because of being sensitive to chemotherapy and radiotherapy Conclusion Primary gastric malignant lymphoma is very likely to be misdiagnosed or neglected due to its rarity and specificity . Operative resection may be impeded by its huge dimension . Better prognosis is often expected if followed by chemotherapy ±radiotherapy .
