准分子激光屈光性角膜切削术治疗近视的疗效分析

摘　　要：目的　评价准分子激光屈光性角膜切削术 (PRK)治疗近视的疗效。方法　应用SchwindkeratomF型准分子激光机对近视屈光度为 - 1.75～ - 12 .0 0D的患者 (30 2眼 )行PRK治疗。按术前球镜屈光度分为两组 :Ⅰ组为- 1.75～ - 6 .0 0D(186眼 ) ,Ⅱ组为 - 6 .2 5～ - 12 .0 0D(116眼 )。随访一年以上。结果　术后 12个月 ,裸眼视力≥ 0 .5者在Ⅰ、Ⅱ组中分别为 96 .7%、90 .5 % ,≥ 1.0者分别为 86 .6 %、6 9% ,Ⅰ、Ⅱ组中屈光度在± 1.0 0D以内者分别为 94.1%、73.3%。并发症主要有角膜雾状混浊 (Haze) ,Ⅰ、Ⅱ组中分别有 0 .5 %、12 .1%的术眼发生了 2级以上的Haze;激素性高眼压在Ⅰ、Ⅱ组中分别占 3.75 %、11.2 %。结论　PRK治疗低、中度近视效果良好 ,对高度近视预测性较差 ,有待于进一步完善手术方法和术后用药。Objectives To evaluate the efficacy of excimer laser photorefractive keratectmy (PRK) for the treatment of myopia.Methods PRK using Schwind keratom F excimer laser was performed with the aim to correct myopia from 1.75 to 12.00 diopters.According to the preoperative myopia,302 eyes were divided into group Ⅰ(≤-6.00D,186 eyes),Ⅱ(-6.25D～-12.00D,116 eyes).All the cases were followed up for more than one year.Results The rates of uncorrected visual acuity ≥0.5 or 1.0 were 96.7%,86.6% in group Ⅰ,and 90.5%,69% in group Ⅱ respectively.94.1% eyes were within ±1.00D of the targeted refraction in group Ⅰ,but 73.3% in group Ⅱ.Corneal haze ≥ 2 grades was 0.5%,12.1% in two groups respectively.Elevated intraocular pressure induced by steroid was observed 3.75% in group Ⅰ,11.2% in group Ⅱ.Conclusions The excimer laser PRK is capable of correcting low and moderate myopia with a relatively high degree of accuracy and safety.However,the predictability and stability for high cases are not so satisfactory.The surgical methods and postoperative administration remain to be further studied.
