摘 要:目的探讨HPV—18病毒E2与E6基因在宫颈病变组织中的表达及其在宫颈癌变过程中的作用。方法采用逆转录聚合酶链反应技术,检测HPV-18阳性的宫颈炎15例、CINI19例、CINⅡ17例、CINⅢ20例、宫颈浸润癌24例的标本中E2及E6基因的表达,比较两者在各组中的表达差异。结果HPV-18E2在宫颈炎、CINI、CINⅡ、CINⅢ和宫颈浸润癌的阳性表达率分别为93.3%、94.7%、58.8%、20.0%及0%;E6在上述各组的阳性表达率分别为6.7%、5.3%、41.2%、85.O%及100%;E2和E6在宫颈炎和CINI的表达差异无显著性(P〉0.05);E2和E6的表达在CINI、CINII、CINⅢ和宫颈浸润癌组间的比较,差异有显著意义(P〈0.05);E6和E2的表达呈负相关(r=-0.728,P〈0.01)。结论HPV-18E2的表达缺失和E6的过度表达,可能是HPV诱导宫颈细胞恶性转化的关键环节,在宫颈癌的演变过程中起重要作用。检测HPV-18E2与E6的表达对早期评估CIN的预后可能有一定的参考价值。Objective To investigate the expressions of human papillomavirus-18 E2 and E6 oncogene in cervical lesions and its correlation with cervical carcinogenesis. Methods The expressions of HPV-18 E2 and E6 gene in the tissues from 15 cases of cervicitis, 19 cases ofCIN I , 17 cases ofCIN Ⅱ , 20 cases of CIN Ⅲ, and 24 cases of invasion cervical carcinoma with HPV- 18 infection were detected by reverse transcription polymerase chain reaction. Results The positive rate of E2 gene expression in groups of cervicitis, CIN I , CIN Ⅱ , CIN Ⅲ and invasion cervical carcinoma were 93.3 %, 94.7 %, 58. 8%, 20.0% and 0% respectively, and that of E6 gene were 6.7%, 5.3%, 41.2%, 85.0% and 100% respectively. No significant differences of E2 and E6 gene expressions were found between groups of cervicitis and CIN Ⅰ ( P〉 0.05 ). However, there were significant differences between any two groups of CIN I , CIN Ⅱ , CIN Ⅲ and invasion cervical carcinoma ( P 〈 0.05 ). The expression of E6 gene was inversely correlated with expression of E2 gene ( r= -0.728, P 〈 0.01 ). Conclusions Deletion of HPV-18 E2 and overexpression of E6 which has been regarded as a key step in malignant transformation of cervical cells inducted by HPV may play an important role in cervical carcinogenesis. Detection of levels of HPV- 18 E2 and E6 expression might be clinically valuable in the prediction of cervical carcinogenesis and the evaluation of prognosis in CIN.
