骨折住院患者自杀态度与焦虑抑郁的相关分析

摘　　要：目的分析骨折住院患者自杀态度与焦虑抑郁的相关性,为骨科临床心理干预提供依据。方法对128例骨折住院患者,选用自行设计的骨折患者一般情况调查表、自杀态度问卷(QSA)、焦虑自评量表(SAS)、抑郁自评量表(SDS)进行问卷调查和测试,以89例健康人为对照组进行对照研究。结果骨折住院患者对自杀态度的四个因子均持矛盾和中立的态度:四个维度均分F1=3.247±0.538;F2=2.864±0.411;F3=2.583±0.528;F4=2.641±0.815。骨折组与对照组在对待自杀行为性质的认识(F1因子)和安乐死的态度(F4因子)差异有统计学意义(P<0.01);骨折患者焦虑、抑郁水平明显高于对照组(P<0.01);相关分析表明,骨折患者对自杀者的态度(F2因子)与其焦虑、抑郁呈正相关(r=0.192和r=0.177,P<0.05);对安乐死的态度(F4因子)与其焦虑呈正相关(r=0.217,P<0.05)。结论骨折住院患者在创伤后焦虑、抑郁水平升高,对自杀普遍持不赞成的态度且其求生、康复期待强烈。提示骨折患者需要临床心理干预和心理护理,而且干预的内容及方式要有针对性。Objective To analyze the correlations of suicide attitude with anxiety and depression in fracture inpatient,in order to provide basis for orthopedic clinical psychological intervention.Methods One hundred and twenty-eight inpatients with fracture were selected as object and were investigated with the self-designed general questionnaire,suicide attitude questionnaire（QSA）,self-rating anxiety scale（SAS） and self-rating depression scale（SDS）.Meanwhile 89 healthy subjects were selected as the control group.Results There were significantly different attitudes to suicide behavior（F1） and to euthanasia（F4） between the fracture group and the control group（P0.01）.Level of Anxiety and Depression were very high in patients with fracture.Significant differences were found in mean score of anxiety and depression between the fracture group and the control group（P0.01）.Correlation analysis indicated that attitude to person who committed suicide（F2） was positively correlative with anxiety and depression and F4 was positively correlative with anxiety.Conclusions It is concluded that the levels of anxiety and depression are very high in fracture inpatients,most of the patients hold disapproval attitudes toward suicide,as well as they have strong desires for living and rehabilitation.So it is necessary to do clinical psychological intervention well for orthopedics,focusing on the content and way of intervention.
