晶状体射流乳化与常规超声乳化对角膜内皮细胞计数影响的Meta分析

摘  要:目的系统评价白内障手术晶状体射流乳化(Fluid-based system)与常规超声乳化(Ultrasound phaco-emulsification)对角膜内皮细胞计数的影响。方法对多个数据库及相关资源进行检索,筛选文献,提取研究所需信息。根据检验假设相同,而假设检验方法不同,选择合并P值法,即χ2=-2∑ln(pi),v,进行Meta分析。结果严格按照纳入与排除标准,共有4篇文献入选,Hu 2010(英国),Richard 2008(法国),Tsorbatzoglou 2007(匈牙利),Sandoval 2006(美国),两种术式术后角膜内皮细胞计数差异的P值分别是,P=0.28,P<0.001,P=0.99和P>0.05,合并P值,χ2=21.76,P<0.001。结论晶状体射流乳化对术后角膜内皮细胞计数的影响显著低于常规超声乳化术。白内障手术中选择晶状体射流乳化术对角膜内皮细胞更安全。Objective To compare in vivo corneal endothelial cell density after fluid-based versus ultrasound phacoemulsification.Methods A comprehensive electric search and a manual search in related resource in English and Chinese literature.Strictly include and exclude the relevant articles with statistical criterion.Formular χ2 =-2∑ln(pi) was used in the meta-analysis.Results 4 articles were included.Endothelial cell density differences were P=0.28 in Hu 2010,P〈0.001 in Richard 2008,P=0.99 in Tsorbatzoglou 2007 and P〈0.05 in Sandoval 2006.In the meta-analysis,χ2 was 21.76,P〈0.001.Conclusion The fluid-based method was safer than conventional ultrasound in cataract surgery.endothelial cell loss after cataract extraction and intraocular lens implantation using a fluid-based system is less than conventional ultrasound.
