摘  要:目的探讨采用实时荧光定量聚合链反应(FQ-PCR)和流式细胞术(FCM),检测乳腺癌患者外周血中CK-19的表达情况及其临床意义。方法用FQ-PCR和FCM方法检测98例Ⅱ~Ⅲ期乳腺癌患者外周血中CK-19的表达情况,另取同期52例乳腺良性肿瘤手术患者作为对照组。结果采用FQ-PCR术和FCM法检测乳腺癌组患者CK-19的阳性率分别为39.8%(39/98)和28.6%(28/98);对照组未检测到CK-19的阳性表达。两种方法及两组间比较,差异均有统计学意义(P<0.05)。激素受体阳性患者CK-19的阳性率分别为39.6%(21/53)和24.5%(13/53),与阴性患者间差异无统计学意义(P>0.05)。HER-2阳性患者检测CK-19的阳性率分别为42.9%(12/28)和35.7%(10/28),与HER-2阴性患者间差异无统计学意义(P>0.05)。有淋巴结转移者CK-19的阳性率分别为49.1%(28/57)和36.8%(21/57),与无淋巴结转移患者差异有统计学意义(P<0.05)。Ⅲ期CK-19的阳性率分别为55.6%(22/36)和41.7%(15/36),高于Ⅱ期的30.6%(19/62)和21.0%(13/62),差异有统计学意义(P<0.05)。结论 FQ-PCR检测微转移的敏感性高于FCM。CK-19的表达与淋巴结转移和肿瘤分期有关,与激素受体、HER-2表达无关。Objective Detect the expression of cytokeratin 19(CK-19) in peripheral blood of breast cancer patients by fluorescence quantitative polymerase chain reaction(FQ-PCR)and flow cytometry(FCM),in order to evaluate the sensitivity in detection of micrometastasis,and its clinical significance.MethodsPeripheral blood samples were analyzed in 98 patients with breast cancer(stage Ⅱ-Ⅲ).52 patients of breast benign tumor were as the control group.FQ-PCR and FCM were used to detect the expression of cytokeratin 19 in peripheral blood.Results The positive rate of CK-19 in breast cancer group was 39.8%(39/98) by FQ-PCR and 28.6%(28/98) by FCM nespectively,while control group turned out to be negative.There were significant difference between the tow groups(P0.05) and the two methods(P0.05).The positive rate of CK-19 in ER(+) and /or PR(+) patients was 39.6%(21/53) by FQ-PCR and 24.5%(13/53)by FCM nespectively.There was no significant difference with positive rate of ER(-) and PR(-) patients(P0.05).The positive rate of CK-19 in HER2(+)patients was 42.9%(12/28) by FQ-PCR and 35.7%(10/28) by FCM nespectively.There was no significant difference with positive rate of HER2(-) patients(P0.05).The positive rate of CK-19 in patients who had metastasis in lymph nodes was 49.1%(28/57)by FQ-PCR and 36.8%(21/57) by FCM nespectively,which had significant difference with patients who didn′t have metastasis in lymph nodes(P0.05).The positive rate of CK-19 in patients of stage was 55.6%(22/36) by FQ-PCR and 41.7%(15/36) by FCM nespectively.It′s higher than in patients of stage Ⅱ which was 30.6%(19/62),21%(13/62).There was significant difference in positive rate of CK-19 between the two stages.(P0.05).Conclusions The sensitivity of FQ-PCR is higher than FCM in detection of micrometastasis.The expression of CK-19 has no relation to ER,PR and HER-2 status,but has relation to metastasis in lymph nodes and stage.
