摘　　要：目的:探讨多排螺旋CT(MDCT)在肠道疾病的临床应用。方法:回顾性分析经明确诊断的63例做过MDCT的肠道疾病患者,通过多平面重建(MPR)、容积重建(VR)及最大密度投影(MIP)等重建方法,观察肠道疾病的MDCT表现。结果:结肠肿瘤22例,小肠肿瘤5例,表现肠壁不规则增厚或分叶性肿块,部分近端肠腔扩张,或肠壁黏膜异常强化。肠梗阻16例,表现为近端肠腔扩张,远端肠腔萎陷,肠扭转还可见鸟嘴征。炎症11例,表现肠壁增厚,强化呈靶征,周围脂肪密度增高,部分可见渗出及包块,周围血管扩张呈梳征。肠系膜转移5例,表现肠系膜密度明显增高,并可见多发或弥漫性结节影,呈网膜饼征。其他4例,包括腹茧症2例、肠系膜上动脉附壁血栓1例、胃结肠瘘1例。结论:MDCT能全景式、全方位展示肠道、肠系膜及血管,对肠道疾病的准确诊断有重要意义。Purpose： To retrospectively investigate the value of multidetector CT（MDCT） and reconstruction techniques in the diagnosis of intestinal diseases.Methods： Sixty-three cases with intestinal diseases underwent MDCT scan of abdomen.CT angiograms were reconstructed with multiplannar reconstruction（MPR）,volume rendering（VR） and maximum intensity projection（MIP）.Results： There were 22 cases of colon tumor and 5 cases of small intestinal tumor.The tumors were with the CT signs of irregularly thickening wall,lobular mass and obstruction in proximal terminal.There were 16 cases of intestinal obstruction,in which the intestinal walls were dilated obviously in proximal terminal while collapsed in distal terminal,and the transformation zone became beak-like narrow in some cases of volvulus.There were 11 cases of intestinal inflammation,in which contrast materialenhanced MDCT demonstrated the thickening wall like a target,and some cases demonstrated extravasation,inflammatory mass and the comb-like mesenteric veins.There were 5 cases of mesentery metastasis,in which MDCT demonstrated hyperattenuating signs and diffuse nods of the mesentery.Conclusion： MDCT can display the whole small intestine and mesentery as well as mesenteric veins clearly,and should be used widely in clinical practice in diagnosis of small intestine diseases.
