绿激光膀胱肿瘤汽化术180例报告

摘　　要：目的:探讨经尿道绿激光膀胱肿瘤汽化术的手术方法、近期疗效及与其它文献方法的比较。方法:采用骶管或硬膜外腔阻滞麻醉,应用美国Laserscope公司生产的非接触式绿激光治疗系统,以30～80W功率对膀胱肿瘤进行汽化。结果:本组汽化时间10～70min,耗能12.23～86.70kJ,出血0～10 ml,术后113例未留置尿管,余67例留置尿管24～96h,膀胱冲洗0～72h,术后住院3～9d,术中均未输血,均未转为开放手术。结论:绿激光膀胱肿瘤汽化术手术安全、操作简单、手术时间短、出血少、痛苦小、留置尿管时间短、康复快,无死角、盲区,可重复性强,是一种理想的微创手术方法。Objective：To evaluate the method of operation and the curative effect in the near future of greenlight vaporization in bladder carcinoma,and compare with other methods.Methods： A total of 180 patients with bladder carcinoma were treated with Niagara PVTM system under sacral anesthesia or epidural anesthesia in 30-80W powers.Results： The operation durations was 10-70 min and cost energy was 12.23-86.70 kJ,and bleeding was 0-10 ml.No postoperative catheterization needed in 113 patients and the catheterization time was 24-96h in remaining 67 patients.The postoperative irrigation time 0-72 h.The postoperative hospital stay time was 3-9 d.No patients required blood transfusion.All didn’t turn into open surgical operation.Conclusions： The green light vaporization of bladder carcinoma is minimal invasion and ideal method of operation.It is characterized by safety,simple,shorter operative duration,few blood loss,little suffering,shorter catheterization,recover quickly and it has no dead angle and blind area.It may be applied repeatly.
