颌面部犬咬伤38例临床治疗体会

摘　　要：目的:总结颌面部犬咬伤的治疗方法及经验。方法:收集2007年6月～2011年5月38例颌面部犬咬伤病例的临床资料,分别采用初期清创与免疫治疗、彻底清创缝合、预防感染、继续免疫接种治疗。结果:35例患者创口I期愈合,3例术后感染延期愈合,1例局部畸形行I I期整复手术,随诊期间均无狂犬病和破伤风发生。结论:犬咬伤患者应尽早注射狂犬疫苗、破伤风抗毒素,强调采用彻底清创美容缝合与预防使用抗生素的联合综合治疗措施。Objective To tally up the methods and experience of emergency treating maxillofacial dog bite cases. Methods The wounds of 38 cases with maxillofacial dog bites were primary debrided and immunized, thoroughly debrided and sutured the wounds,then prevented infection and specific immunized. Results The wounds were healed in stage-I in 35 cases,infected postoperatively and postponed to heal in 3 cases,local deformity was repaired in stage- II in 1 case.No case was suffered from rabies and tetanus. Conclusion It is emphasized that the cases with maxillofacial dog bite should be immunized to reduce the rate of rabies and tetanus occurrence earlier,and the comprehensive measures of thorough debridement and suture in conjunction with antibiotic prophylaxis should be done.
