吸烟对急性脑梗死患者血清sVCAM-1和TNF-α水平的影响及临床意义

摘　　要：目的 通过检测吸烟脑梗死患者血清可溶性血管细胞黏附分子-1（soluble vascular cell adhesion molecule-1,sVCAM-1）,肿瘤坏死因子-α（tumor necrosis factor-α,TNF-α）水平变化,探讨吸烟对脑梗死的影响.方法 采用双抗体夹心酶联免疫吸附法（ELISA））测定47例吸烟脑梗死患者、49例不吸烟脑梗死患者,45例吸烟健康者和42例不吸烟健康者血清sVCAM-1和TNF-α水平.结果 脑梗死患者血清sVCAM-1,TNF-α水平明显高于无脑梗死者（P＜0.01）;吸烟脑梗死患者明显高于不吸烟脑梗死患者（P＜0.01）;吸烟健康者明显高于不吸烟健康者（P＜0.01）.重度吸烟组血清sVCAM-1,TNF-α水平明显高于中度吸烟组（P＜0.05）、轻度吸烟组（P＜0.01）和健康不吸烟组（P＜0.01）;中度吸烟组明显高于轻度吸烟组（P＜0.05）和健康不吸烟组（P＜0.01）.吸烟指数与sVCAM-1,TNF-α水平呈正相关.结论 吸烟使脑梗死患者和健康者sVCAM-1,TNF-α水平升高.Objective To investigate the changes of serum soluble vascular cell adhesion molecule- 1（sVCAM-1） and tumor necrosis factor-α （TNF-α） concentration among healthy smokers and cerebral infarct patients,and to study the effects of smoking on the occurrence and development of cerebral infarct.Methods The levels of serum sVCAM-1 and TNF-α were measured by two-layer antibody sandwich enzyme-linked immunosorbent assay（ELISA） in 47 cases of smokers with cerebral infarct and 49 cases of non-smokers with cerebral infarct,45 healthy smokers and 42 healthy non-smokers.Results The level of serum sVCAM-1 and TNF-α in the patients of cerebral infarct was significantly higher than that in the person of non-cerebral infarct（P 〈 0.01）,and the level of smokers with cerebral infarct was higher than that of non-smokers with cerebral infarct（P 〈 0.01）;the level of healthy smokers＇ was higher than that of healthy non-smokers＇（P 〈 0.01）.The level of serum sVCAM-1 and TNF-α in the severity smoker was significantly higher than that of mild（P 〈 0.01） and moderate（P 〈 0.05） smoker and healthy non-smoker（P 〈 0.01）.The level of serum sVCAM-1 and TNF- α was well correlated with smoking index.Conclusion Smoking can elevate the sVCAM- 1 and TNF- α level of the health and cerebral infarction patients.
