舒必利合并奥氮平治疗难治性精神分裂症对照研究

摘　　要：目的比较舒必利合并奥氮平治疗难治性精神分裂症的疗效及安全性。方法将45例难治性精神分裂症患者随机分为两组：舒必利合并奥氮平为研究组（n=22）和奥氮平对照组（n=23）,于治疗前和治疗后1、2、4、8、12周末进行阳性和阴性症状量表（PAN SS）及不良反应症状量表（TESS）评价疗效及副反应。结果治疗第2周末研究组总分、阳性及阴性症状,一般病理分比治疗前明显降低（t=2.86-4.99,P〈0.0.1）,而对照组于第4周才起效,且研究组的总分,阴性症状分及所用奥氮平剂量明显低于对照组（t=2.24-3.27,P〈0.05或P〈0.01,t=5.088,P〈0.01）。结论舒必利合并奥氮平对难治性精神分裂症具有起效快,疗效好,副反应少,可降低病人费用负担,增加长期服药依从性。Objective To observe the effectiveness and safety of the Dogmatil combining Olanzapine in the treatment of resistant schizophrenia.Methods A total of 45 cases of resistant schizophrenia were randomly divided into two groups,namely study group was Donmaitl combining Olanzapine group with 22 cases and the control group was Olanzapine group with 23 cases.Two groups were compared by PANSS and TESS to evaluate the effectiveness and side effects at the time-points of pre-therapy,and 1 week,2 weeks,4 weeks,8 weeks and 12 weeks after therapy.Results After 2-week therapy,the total score,positive and negative symptoms and common ill score reduced significantly compared to pre-therapy in study group（t=2.86~4.99,P0.0.1）,but in control group,those happened after 4 weeks.And the team total score,negative symptoms points and the olanzapine dose were significantly lower in study group than in control group（t=2.24~3.27,P0.05 or 0.01,t=5.088,P0.01）.Conclusion Donmaitl combining Olanzapine will have quick and good effect on resistant Schizophrenia but few side effects,and reduce medical expenses and increase long-term adherence.
