摘 要:目的评价小切口非超声乳化白内障摘除术对角膜内皮的影响。方法临床病例对比研究。收集2010年1月至2011年6月就诊于福建医科大学附属第一医院眼科的白内障患者123例(136只眼),由同一位医师主刀完成小切口非超声乳化白内障摘除术,术前和术后1周、1月及3月采用角膜内皮镜测量角膜上部、中央区和下部的角膜内皮,计算角膜内皮细胞的密度、丢失率、六边形细胞比例和细胞面积。结果术前角膜上部、中央区和下部的角膜内皮细胞密度、六边形细胞比例和角膜内皮细胞面积差异均无统计学意义(均P〉0.05)。术后角膜内皮细胞密度下降、六边形细胞比例下降、细胞面积扩大。角膜上部、中央区和下部的角膜内皮细胞密度、细胞丢失率、六边形细胞比例和角膜内皮细胞面积差异以1周时差别最大,差异有统计学意义(均P〈0.01);1月时差异有统计学意义(均P〈0.05);3月时差异无统计学意义(均P〉0.05)。结论小切口非超声乳化白内障摘除术对角膜内皮细胞损伤小,损伤部位以角膜上部切VI处为主。Objective To investigate the effect of small incision nonphacoemulsification cataract surgery on corneal endothelial cells. Methods One hundred and twenty-three cataract patients (136 eyes) were collected from January 2010 to June 2011 in our hospital. Small incision nonphacoemulsi- fication cataract extraction was performed by the same physician. The corneal endothelial cells on the upper, the central and the lower corneal were measured by using corneal endothelial microscope preoperative and 1 week, 1 month and 3 months postoperative. The corneal endothelial cells density, loss rate, hexagonal cell ratio and cell area were calculated. Results The differences among the up- per, the central and the lower cornea of corneal endothelial cells density, hexagonal cell ratio and cell area were no statistical significances (all P 〉0.05) before operation. The corneal endothelial cells density decreased, the hexagonal cells ratio decreased and the cell area increased after operation. The differences among the upper, the central and the lower cornea of corneal endothelial cells densi- ty, hexagonal cell ratio and cell area were the largest at 1 week after operation and were statistical significances (all P 〈0.01). The differences among the upper, the central and the lower cornea of cor- neal endothelial cells density, hexagonal cell ratio and cell area were statistical significances at 1 month after operation (all P 〈0.05). The differences among the upper, the central and the lower cor- neal of corneal endothelial cells density, hexagonal cell ratio and cell area were no statistical signifi- cances at 3 months after operation (all P 〉0.05). Conclusions Small incision nonphacoemulsifica- tion cataract extraction has a small amount of damage on corneal endothelial cells, and the damage position is mainly at the incision on the upper part of corneal.
