不同戴镜情况近视患者LASIK术后立体视功能的研究

摘　　要：目的探讨准分子激光原位角膜磨镶术（LASIK）对不同戴镜情况的近视患者立体视功能的影响。从双眼视功能的角度评价LASIK手术的疗效，为术后视觉质量的预测提供临床依据。方法入选对象为2008年3月到2008年8月间行LASIK治疗双眼近视及近视散光的患者84例，选取屈光正视者30例作为对照组，年龄18-33岁，平均26．7岁，按等效球镜度分为低度近视组、中度近视组和高度近视组。根据术前戴镜状况分为持续戴镜组、间断戴镜组和未戴镜组。术后随诊3个月，比较不同屈光度的不同戴镜组术前裸眼、术前戴镜、术后10d及术后三月的远近立体视功能。结果术前戴镜检查，间断戴镜组、未戴镜组与对照组相比，近、远立体视功能受损（P〈0．05）。高度近视持续戴镜组与对照组相比，近、远立体视功能受损（P〈0．05）。术后，不同屈光度三组的近、远立体视功能得到改善，但中、高度近视患者，间断戴镜组和未戴镜组未完全恢复正常。结论近视患者不坚持配戴合适的眼镜会出现立体视功能受损。LASIK术后立体视功能渐趋正常，特别是对于间断戴镜组、未戴镜组的近视患者，有利于恢复立体感知。Objective To investigate the effects on stereopsis after LASIK for myopic patients with different habits of wearing glasses, in order to evaluate the clinic value of LASIK from binocu- lar vision function. Methods Thirty emmetropia and 84 myopia and myopia astigmatism who want- ed LASIK from March 2008 to August 2008, aged 18-33 years old were selected from clinic. Eighty-four myopia were divided into 3 groups by equivalent spherical power. According to status of wearing glasses, they were divided into three groups of continued wearing glasses, interrupted wear- ing glasses and not wearing glasses. Stereopsis acuity and diopters were compared before and 3 months after surgery in turn according to their status of wearing glasses. Results On the status of wearing glasses, there were significant difference among interrupted wearing glasses, not wearing glasses and emmetropia preoperatively in near and distant stereopsis （P 〈0.05）. There were signifi- cant difference among emmetropia and continued wearing glasses preoperatively on the status of wearing glasses in distant and near stereopsis with high myopia （P 〈0.05）. Three months after sur- gery, the near and distant stereopsis were improved in three groups, but not recovered completely in interrupted wearing glasses and not wearing glasses with medium and high myopia. Conclusions Uncontinued .wearing glasses would damage their stereopsis. LASIK may effectively correct general myopia. Postoperatively, LASIK may accelerate the formation of ability of stereopsis vision, espically for myopia with interrupted wearing glasses and not wearing glasses.
