海洛因成瘾患者强制隔离戒毒期满入组美沙酮维持治疗的影响因素分析

摘　　要：目的探讨影响海洛因成瘾患者强制隔离戒毒期满入组美沙酮维持治疗门诊的影响因素。方法运用队列研究的方法,在强制隔离戒毒所海洛因成瘾患者出所前问卷调查了解患者一般情况、家庭状况、吸毒史、心瘾状况、接受美沙酮维持治疗相关知识宣传的状况,随访患者出所后6个月是否入组MMT,将结果进行成组Logistic回归分析。结果 2010年3月-2011年3月期间出所198人,2009年1-12月期间出所200人,追踪6个月,结果入组MMT患者53人,成组Logistic回归结果显示:婚姻状态、宣传情况、年龄段、居住状态、家庭关系、生育史影响海洛因成瘾患者入组MMT。单身的病例较在婚的病例进入MMT的OR值为609.03,接受宣传的病例较未接受宣传的病例进入MMT的OR值为14.92;年龄每增加一个等级进入MMT的OR值为0.50,独居者对与父母、妻儿同居者进入MMT的OR值为0.05,家庭关系紧张者对关系一般、融洽者进入MMT的OR值为0.06,未生育者对生育者进入MMT的OR值为0.18。结论强制戒毒期满出所的海洛因成瘾患者在婚、未接受专业宣传、年龄大、独居、家庭关系紧张、未生育者,不容易进入MMT治疗。MMT门诊可采取综合措施增加患者出所后的参治率,让患者接受专业的MMT宣传、让患者与亲人同住、改善患者的家庭关系等,提高美沙酮维持治疗门诊的覆盖率。Objective To explore the impact factors of heroin addicted patients joined in methadone maintenance treatment(MMT) after compulsory isolation rehabilitation.Methods A six-month cohort questionnaire survey was conducted among heroin addicted patients with compulsory isolation rehabilitation to know about their basic situations,family status,drugs history,spirit addiction status,accepted status of MMT publicity so as to assess the rate of joining in MMT,and the results were estimated by Logistic regression analysis.Results Logistic regression analysis on 53 patients involved in MMT among 398 patients(including 198 patients with rehabilitation from March 2010 to March 2011 and 200 patients with rehabilitation from January to December,2009) showed that: the impact factors for heroin addicted patients to join in MMT were marital status(OR=609.03,P<0.05),publicity situations(OR=14.92,P<0.05),age groups(OR=0.50,P<0.05),living status(OR=0.05,P<0.05),family relationship(OR=0.06,P<0.05),and childbearing history(OR=0.18,P<0.05).Conclusions Heroin addicted patients after compulsory isolation rehabilitation are unlikely to join in MMT on the situations of being married,un-receiving professional propaganda,older ages,living alone,tense family relationship,and nulliparae.MMT clinics can adopt comprehensive measures in the patients,including accepting professional MMT propaganda,living with family members,and improving family relationship,so as to increase the rate of patients' receiving MMT after compulsory isolation rehabilitation and the coverage rate of MMT.
