华佗再造丸联合针刺及康复训练对社区脑卒中患者康复疗效评价

摘　　要：目的评价华佗再造丸联合针刺及康复训练对社区恢复期脑卒中患者运动功能、日常生活能力的影响,证实中医药综合方案的有效性。方法选择脑卒中住院治疗后进入社区中医辨证为痰瘀阻络证的恢复期病人110例,试验组64例,对照组46例,2组病人均给予社区基础康复治疗。试验组加用中医药综合康复方案。2组患者治疗前、治疗1个月末及随访1个月末采用神经功能缺损评分、改良Barthel指数积分进行评价。结果治疗1个月末和随访1个月末较治疗前神经功能缺损评分下降(P<0.05),改良Barthel指数积分升高(P<0.05)。试验组与对照组比较治疗1个月末神经功能缺损评分及改良Barthel指数积分无统计学意义(P>0.05);随访1个月末试验组较对照组神经功能缺损评分下降明显(P<0.05),改良Barthel指数积分升高明显(P<0.05),试验组临床疗效优于对照组(P<0.05)。结论华佗再造丸联合针刺及康复训练可改善社区脑卒中恢复期患者的神经功能,提高日常生活能力。Objective To evaluate the effect of Huatuo Zaizao Boluses combined acupuncture and community rehabilitation training on motor function and ADL of convalescent stroke patients and demonstrate effectiveness of the traditional Chinese medicine of comprehensive approach. Methods One hundred and ten community convalescent stroke patients after hospitaliza- tion were selected, whose TCM syndromes were measured up to deficiency of phlegm-stasis in channel. They were divided into the experimental group （n = 64） and control group （n = 46）. Patients in the two groups received community- based rehabilitation treatment. The experimental group patients combined with the traditional Chinese medicine of comprehensive approach, inclu ding traditional Chinese drugs（TCD） and acupuncture. Before treatment, Post-treatment and one month Dost-treatment, all patients were assessed with neurological impairment score and modified Barthel index（MBI）, and the effect of two groups was compared. Results Post treatment and one month after treatment compared with before treatment, neurological impairment of two groups score declined, MBI improved（P〈0.05）. After treatment, parameter had no significant difference between the two groups （P〉0.05）. The improvement of treatment group was higher than that of control group one month after treatment（P〈 0.05）, and was more significant in the treatment group than that in the control group（P〈0.05）. Conclusion Huatuo Zaizao Boluses combined acupuncture and community rehabilitation training can improve neurological impairment and ADL of commu nity convalescent stroke patient.
