常见中医疗法治疗椎动脉型颈椎病临床对比观察

摘　　要：目的探讨5种常见中医疗法治疗椎动脉型颈椎病的临床疗效。方法将150例患者按随机数字表法随机分为中药内服组、中药外敷组、针刺组、耳穴贴压组和推拿组5组,每组30例,观察每组治疗前后的临床疗效和椎-基底动脉系统血流动力学指标变化。结果 5组中医治疗方法针刺法和推拿法效果最好。对5组中医治疗方法间的治愈率和总有效率进行两两比较,中药外敷组与针刺组(P<0.05)、推拿组(P<0.05),耳穴贴压组与针刺组(P<0.05)、推拿组(P<0.05)在治愈率上存在差异;耳穴贴压组与针刺组(P<0.05)、推拿组与耳穴贴压组(P<0.05)在总有效率上存在差异。通过椎-基底动脉系统血流动力学分析,针刺法和推拿法可以对左右椎动脉的血流供应产生一定的作用(P<0.05),并且针刺法还可以在一定程度上改善血管搏动(P<0.05);中药内服法可以对左右基底动脉产生影响(P<0.05);而中药外敷和耳穴按压法则会对左右椎动脉的平均流速产生作用(P<0.05)。结论针刺法和推拿法在5种中医治疗椎动脉型颈椎病的方法中优于其他3种方法,可以初步判定为中医临床治疗椎动脉型颈椎病优选方案。OBJECTIVE To discuss the clinical effect of the five usual Chinese medicine therapy methods to treat cervical spondylosis of vertebral artery type.METHODS 150 cases were randomly divided Into 5 groups,including the herbal internal and external treatment group,Chinese traditional medicine group,acupuncture group,ears stick pressure group and massage.Every group have 30 cases,observed each group the clinical curative effect and the changes of vertebral artery system＇s basement hemodynamic parameters before and after treatment.RESULTS In the five groups,the acupuncture group and the massage group had the best effect.Then proceed multiple comparison between the cure rate and total effective rate in the five TCM treatment methods groups.Chinese medicine external group and acupuncture group（P〈0.05）、massage group（P〈0.05）、ears stick pressure group and acupuncture group（P〈0.05）、massage group（P〈0.05）had differences in the cure rate.Ears stick pressure group and acupuncture group（P〈0.05）、massage group and ears stick pressure group（P〈0.05）had differences in the total effective rate.From the analysis of vertebralartery system＇s basement hemodynamic parameters,acupuncture and massage methods can have certain effect to the Vertebral artery＇s blood supply（P〈0.05）.Also,acupuncture improved blood vessel beating to a certain extent.Chinese medicine internal method had an affect on the vertebral artery（P〈0.05）;even Chinese medicine external and ears stick pressure methods had an affect on the average flow velocity of the Vertebral artery（P〈0.05）.CONCLUSION The acupuncture and massage methods are better than the other three methods.In Chinese traditional treatment of cervical spondylosis of vertebral artery type.We can initially judge acupuncture and massage methods as optimization in Chinese clinical treatment of cervical spondylosis of vertebral artery type.
