摘 要:目的探讨溃疡性结肠炎(UC)与肠易激综合征(IBS)患者肠黏膜组织中$100阳性表达的树突状细胞(S100^+DC)、CD83阳性表达的树突状细胞(CD83^+DC)与白细胞介素(1L)23P19的表达差异。方法以60例UC、60例IBS患者的结肠镜活检组织蜡块标本为研究对象,应用免疫组化SP法检测两种病变肠黏膜组织中S100^+DC、CD83^+DC及IL-23P19的表达情况。结果两种病变组织中S100^+DC、CD83^+DC阳性表达率差异无统计学意义(P〉0.05);IL-23P19阳性表达率差异有统计学意义(85.O%与70.O%,)(2=3.87,P〈0.05);轻型UC组S100^+DC、CD83^+DC及IL-23P19三个指标阳性细胞浸润密度分别为(24.43±8.26)、(5.23±1.66)、(95.03±18.39)个/mm^2,重型UC组分别为(36.13±11.36)、(10.13±3.29)、(133.38±25.32)个/mm^2,两组对应的指标比较差异均有统计学意义(P均〈0.05);全结肠型与局部结肠型UC的三个指标阳性细胞浸润密度比较,差异均无统计学意义(P均〉0.05)。结论UC组织中DC的浸润密度增高且分化能力增强,IL一23P19分泌增多;UC组织中,随着病情的加重S100^+DC、CD83^+DC、IL-23P19阳性表达明显增多。Objective To compare the levels of S100^+ DC, CD83^+ DC and IL-23P19 between the patients with mucosa of ulcerative colitis ( UC ) and those with irritable bowel syndrome ( IBS ). Methods Paraffin-embedded colon tissue from colonoscopie biopsy was detected in sixty UC and sixty IBS cases for the positivity of S100^+ DC, CD83^+ DC and IL-23P19 using immunohistochemical SP method. Results There was no significant difference in the positive rates of S100^+ DC and CD83^+ DC between the.two groups ( P 〉 0. 05 ), while significant difference in IL-23P19 level was found between the two groups ( 85.0% vs. 70. 0% ; X^2 = 3.87, P 〈 0. 05 ). The infiltration densities of S100^+ DC, CD83^+ DC and expression of IL-23P19 in the mild and severe UC patients were 24.43 ± 8. 26/mm2,5.23 ± 1.66/mm^2, 95.03 ± 18. 39/mm^2, and 36. 13 ± 11.36/mm^2, 10. 13 ± 3.29/mm^2, 133.38 ± 25. 32/mm^2, respectively. The difference between the mild and severe UC patients was significant ( P 〈 0. 05 ) . No significant difference was found regarding the above three parameters between severity-stratified subgroups of the UC patients ( P 〉 0. 05 ). Condusion In UC colon tissues, the infiltration density and activity of DC were increased. IL-23P19 level was elevated. The levels of S100^+ DC, CD83^+ DC and IL-23P19 were increased with the advancement of UC disease.
