肝脏局灶性结节增生的螺旋CT诊断

摘　　要：目的:分析肝脏局灶性结节增生(FNH)在螺旋CT多期增强扫描中的表现,以提高CT对FNH的诊断准确性。方法:回顾性分析15例16个病灶经手术病理证实的FNH患者的临床及螺旋CT平扫、动脉期(25s)、门静脉期(60s)、延迟期(180～300s)增强扫描资料。结果:FNH多见于中青年,多无临床症状,主要分布在肝脏表面,病变边界清晰,平扫13个病灶呈略低密度,3个病灶呈等密度者,其中10个病灶内见点状、小星状、小片状低密度中央瘢痕影;增强动脉期扫描所有病灶除中心瘢痕外,均呈明显均匀一致的强化,其中5个病灶还可见到中心或周边增粗、扭曲的动脉;门脉期扫描3个病灶呈高密度,9个病灶呈略高密度,5个病灶呈等密度;延迟期扫描16个病灶均呈略高或等密度;8个病灶中心瘢痕有延迟强化,2个无强化。结论:抓住螺旋CT多期增强扫描中的特征性表现,有助于FNH的诊断及鉴别诊断,减少临床不必要的治疗。Objective： To analyze enhanced spiral CT scan images ot hepatic tocal nodular byperptasia （FNH） in order to improve the accuracy of diagnosis of FNH. Methods： Retrospective analysis of CT scan images of 15 （16 lesions） pathology confirmed FNH patients including plain scan, and enhanced arterial phase （25 s）, portal venous phase （60 s）, delay period （180 - 300 s） were conducted. Results：FNH is more commonly among young people with no obvious clinical symptoms, and it mainly presented in the liver surface with clear lesion boundary. The plain scan showed 13 lesions with slightly low-density, the other 3 lesions with normal density. Dots or small pieces of low-density central scar shadow were revealed in 10 lesions. Under enhanced Ct scan, except the central scar shadow the images were obviously enhanced, of which shadow with increased density or torched veins were revealed in centre or margins of 5 lesions, high density shadow and slightly high density was presented in 3 and 9 lesions at the portal venous phase, 16 at delay period. Central scars of 8 lesions showed delayed enhancement, while 2 didn＇t. Conclusions-Characters of enhanced spiral CT scan images is helpful for the diagnosis and differential diagnosis of FNH thus contributes to prevention of unnecessary treatments.
