重明益损汤治疗气虚血瘀型外伤性视神经萎缩的临床研究

摘　　要：目的观察重明益损汤治疗气虚血瘀型外伤性视神经萎缩的疗效。方法辨证为气虚血瘀证的外伤性视神经萎缩(traumatic optic atrophy,TOA)患者100例,随机分为中药治疗组(50例54只眼)和对照组(50例63只眼),前者予口服重明益损汤,后者予口服甲钴胺(弥可保),2组均静脉滴注灯盏花素注射液。以14 d为1个疗程,共治疗3个疗程,比较2组治疗前后视力、视野、视觉诱发电位和中医证候积分的变化。结果中药治疗组显效12只眼,好转22只眼,总有效率63.0%;对照组显效8只眼,好转20只眼,总有效率44.4%,2组差异有统计学意义(P=0.045)。中药治疗组在视野平均缺损、丢失方差、视觉诱发电位P100波峰潜时,以及中医证候积分方面的改善情况好于对照组,组间差异有统计学意义(P<0.05)。结论重明益损汤对气虚血瘀型TOA患者有一定疗效,在改善视力、视野、视神经传导方面优于口服弥可保治疗。OBJECTIVE To observe the clinical curative effect of Chongming Yisun Decoction on treatment of traumatic optic atrophy（TOA） with syndrome of Qi deficiency and blood stasis.METHODS The study selected 100 patients of TOA with syndrome of Qi deficiency and blood stasis and these patients were randomly divided into experimental group（50 patients 54 eyes） and control group（50 patients 63 eyes）.The experimental group was treated with Chongming Yisun Decoction,and the control group with Mecobalamin（Methycobal）.Both groups were given intravenous Brevoscapine injection.Patients were treated for 3 periods of treatment（14 days as a period）.Before and after treatment,patients＇ vision,visual field,visual evoked potential（VEP）,the TCM symptoms of Qi deficiency and blood stasis were collected and compared.RESULTS The total effective rate of experimental group was 63%,with 12 eyes markedly improved,22 eyes improved.The total effective rate of control group was 44.4%,with 8 eyes markedly improved,20eyes improved.The curative difference had statistical significance（P=0.045）.The improvement of visual field mean defect,visual field loss variance,visual evoked potential latency of P100 wave and the total accumulated points of TCM symptoms of the experimental group were better than the control group,and the difference had statistical significance（P0.05）.CONCLUSIONS Chongming Yisun decoction had a certain curative effect for patient of TOA with syndrome of Qi deficiency and blood stasis and it was better than Methycobal in the aspects of improving vision,visual field and optic nerve conductive.
