结膜新月形切除术与双极电凝术治疗结膜松弛症的临床疗效观察

摘　　要：目的探讨结膜新月形切除术与双极电凝术治疗结膜松弛症的临床效果。方法前瞻性研究。60例（60只眼）结膜松弛症手术病例，随机数字表法分为2组，各30只眼。一组行双极电凝治疗术，另一组行结膜新月形切除术。比较两种手术方法术后眼表面疾病指数（OSDI）积分、结膜松弛的程度、泪河高度、BUT、手术并发症、手术操作时间来评价2种手术方法的临床疗效。结果双极电凝治疗术与结膜新月形切除术术后8周OSDI积分分别为（7．80±2．46）、（8．46±2．36）分，松弛结膜残留积分分别为2分及3分，泪河正常眼比例分别为86．6％（26／30）及93．3％（28／30），BUT≥10s的眼比例分别为56．6％（17／30）及60．0％（18／30），差异均无统计学意义（t=1．47，1．29；χ2=0．741，0．069；P〉0．05）。术后8周双极电凝术并发症积分低比结膜新月形切除术低，差异有统计学意义（t=4．67，P=0．029）；双极电凝手术操作时间（9．53±3．15）min短于结膜新月形切除术的（18．59±7．68）min，差异有统计学意义（t=16．175，P=0．000）。结论结膜松弛症行双极电凝和松弛结膜新月形切除具有相当的疗效，双极电凝手术时间明显缩短、术后并发症减少，手术方法更简便。Objective To seek safe, effective, economical, simple treatment conjunctivoehalasis surgical methods, optimize treatment, evaluation conjunetivochalasis surgical treatment. Methods A prospective randomized control study, 60 patients （60 eyes ） conjunetivoehalasis surgery patients were randomly divided into two groups, one line of bipolar coagulation therapy, another group of crescent conjunctival resection. After comparing the two surgieal methods ocular surface disease index （OSDI） points, the degree of relaxation conjunctiva, tear meniscus height, BUT, surgical complications, the operation time to evaluate the two kinds of surgical methods of clinical efficacy. Results Bipolar coagulation therapy with crescent eonjunetival reseetion in 8 weeks after the OSDI points, loose eonjunctiva residual points, tear meniscus, BUT the difference was not statistically significant. 8 weeks after bipolar coagulation complications points lower than the crescent conjunctival resection is low, the difference was statistically significant （ t = 4. 67, P = 0. 029 ） ; bipolar coagulation operating time （ 9. 53 ± 3.15 ） rain crescent than conjunctival resection time （18. 59 ±7.68 ）rain short, the difference was statistically significant （t = 13.26, P 〉 0. 01 ）. Conclusions Conjunetivoehalasis line bipolar coagulation and removal of loose eonjunetiva crescent with considerable effect, bipolar coagulation was significantly shorter operative time, a significant reduction in postoperative complications, surgical procedures easier.
