摘  要:目的:对脑瘫患儿步态的时空及运动学参数进行可靠性分析,确定不同类型脑瘫患儿步态参数的最佳测试次数。方法:利用三维步态分析系统采集45例脑瘫患儿(26例偏瘫和19例双瘫)步态的时空及运动学参数,采集有效测试8次;利用组内相关系数(Intra-class correlation coefficients,ICC)对步态参数进行可靠性分析。结果:偏瘫患儿重复测试5次、双瘫重复4次可使各时空参数的ICC值均达到0.9以上;偏瘫患儿重复测试6次、双瘫重复5次可使运动学各参数的ICC值均达到0.9以上。结论:脑瘫患儿时空参数的可靠性高于运动学参数,双瘫患儿时空和运动学参数的可靠性均高于偏瘫患儿;偏瘫患儿重复测试6次、双瘫患儿重复测试5次能使其时空和运动学参数达到较高的可靠性。Objective To identify the optimal testing frequency of temporal-spatial gait parameters and kinematic parameters in children with cerebral palsy (CP) through reliability analysis. Methods Three-dimensional gait analysis system was used to collect temporal-spatial gait parameters and kine- matic parameters 8 times from 45 children with CP(26 hemiplegic and 19 diplegic). Intra-class correla- tion coefficients (ICC) was carried out for reliability analysis. Results After 5-time testing in children with hemiplegic cerebral palsy and 4-time in children with diplegic cerebral palsy,the ICC of temporal- spatial gait parameters of them reached beyond 0.9. After 6-time testing in children with hemiplegic cerebral palsy and 5-time in children with diplegic cerebral palsy, the ICC of gait kinematic parameters of them reached beyond 0.9. Conclusions (1)The reliability of temporal-spatial gait parameters is better than that of kinematic parameters in children with cerebral palsy. The reliability of temporal-spatial gait parameters and kinematic parameters of children with diplegic cerebral palsy are better than that of children with hemiplegic cerebral palsy. (2)The optimal reliability of temporal-spatial gait parameters and kinematic parameters could be obtained through 6-time testing and 5-time testing for children with hemiplegic cerebral palsy and children with cerebral palsy respectively.
