背胰发育相关变异的影像学表现

摘　　要：目的 从发育角度阐述背胰变异的影像特点.方法 遴选53例背胰变异病例的CT、MRI资料,分类解释其影像特点.结果 （1）背胰局部突起（20例）：胰腺腹侧或腹外侧局部突起,多位于胰颈;（2）胰尾异常增宽（15例）：横断面图像上,胰尾前后径＞3 cm并明显超过胰体宽度;（3）背胰发育不良（9例）：因胰尾缺如,背胰呈＂短胰＂表现,其中5例合并多脾综合征和腹胰缺如;（4）胰腺脂肪裂（5例）：表现为与胰腺长轴垂直的裂隙样脂肪密度影,其中4例位于胰体胰尾交界处,1例位于腹胰背胰交界处;（5）胰尾分叉（4例）：横断面像上,胰尾增宽,脾静脉穿行其间,其中1例真性分叉者伴有胰管分叉.结论 背胰发育异常并不少见,临床多表现为不伴胰腺炎等并发症的单纯形态异常.Objective To explain the imaging features of developmental variations of dorsal pancreas. Methods CT and MR ima ging data of 53 cases with variations of dorsal pancreas were collected. Imagiug characteristics of dorsal pancreas were analysed. Resuits Processes locally of pancreatic contour （n=20） ： which were local process at the ventral aspect of Imncreas, and al the neck of pancreas were common. （2）Enlargement of cauda pancreatis（n= 15 ） ： anteroposterior diameter of the tail of pancreas was larger than 3 cm and obviously wider than the normal body of pancreas on axial images. （3）Dorsal pancreatic dysplasia （n=9）：due to the lack of eauda pancreatis,the dorsal pancreas appeared as ＂short pancreas＂, five of them complicated with polyspenia syndrome and absence of ventral pancreas. （4）Pancreas separated by fat spatium（n=5）： the crevice like fat was perpendicular to the major axis of the pancreas, which located in the juncture of ventral and dorsal pancreas in one,in the j unct ure of body and tail of the pancreas in 1. Bifurcation of cauda pancreatis（n=4） ： splenic vein crossed in the enlarged tail of panreas on axial image and one of them accompa nied with bifucation of the pancreatic duct, which was called true bifurcation. Conclusion The developmental variations of dorsal pancreas are common, which display mostly simple morphologic abnormality without complications such as pancreatitis in the clinic.
