乳腺浸润性导管癌肝转移的超声造影特性

摘　　要：目的探讨乳腺癌肝转移的超声造影(CEUS)特性。方法回顾性对比分析60例乳腺浸润性导管癌不同大小肝转移灶间增强模式的差异并与相同大小的肝细胞性肝癌(HCC)的增强模式进行比较,同时对肝转移灶与HCC的声学定量时间-强度曲线的相关参数进行比较。结果 58.3%(35/60)乳腺癌肝转移灶动脉相呈周边环状高增强,29例>2 cm的转移灶72.4%(21/29)的环状高增强,31例≤2 cm的转移癌51.6%(16/31)整体均匀性高增强。4例≤1 cm的转移灶100%整体高增强。乳腺癌肝转移灶与HCC的始消时间、增强速率、峰值强度、50%清除斜率比较均P<0.05。结论动脉相快速强化是乳腺癌肝转移的共同特点,但增强模式与转移灶大小有关。环状增强及造影剂廓清更快是乳癌肝转移有别于HCC的CEUS特性。Objective To investigate the characteristics of contrast-enhanced ultrasound （CEUS） in liver metastasis from breast cancer. Methods A retrospective comparative analysis of CEUS imaging was performed between 60 cases of liver metastasis from breast invasive ductal carcinoma and 60 cases of hepatocellular carcinoma （HCC）. The enhancement patterns were observed with different size of metastasis, compared with HCC of the same size, the timeintensity curve was obtained. The arrival time, regression beginning time, peak intensity, enhancement rate and clearance slope of 50% were compared between liver metastasis and HCC. Results Of all 60 metastasis, 35 lesions （58.3%） were hyper-enhanced in rim-like from periphery in arterial phase; In 29 metastasis bigger than 2 cm, 21 lesions （72.4%） were hyper-enhanced in rim-like; In 31 metastasis smaller than 2 cm, 16 lesions （51.6%） showed total homogeneous hyper-enhancement; All of 4 metastasis smaller than 1 cm showed total hyper-enhancement. The regres- sion beginning time, enhancement rate, peak intensity and clearance slope of 50% showed significant difference in liver metastasis from breast cancer and HCC （all P〈0.05）. Conclusions Liver metastasis from breast cancer was manifested as rapid enhancement in arterial phase. The enhancement patterns were related to the size of metastasis. Compared with HCC, liver metastasis from breast cancer showed ring enhancement and regressed quickly.
