弥漫性大B细胞淋巴瘤患者染色体畸变及与临床分期的关系

摘  要:目的明确弥漫性大B细胞淋巴瘤患者常见的染色体畸变及其与临床分期的关系。方法本研究采用改良的染色体制备方法,对25例弥漫性大B细胞淋巴瘤(DLBCL)患者的外周血及骨髓进行染色体分析,探索染色体畸变与临床分期的关系。结果25例外周血染色体标本中,22例获得满意分裂相,其中19例有克隆性异常。染色体异常多涉及1、2、3、5、7、13、14、17、18、22号及x等染色体。制备25例骨髓染色体,19例获得满意分裂相,其中有16例克隆性异常。25例对照组染色体异常核型有1例,与以上两组比较差异均有统计学意义(P〈0.05)。Ⅲ期及Ⅳ期弥漫性大B细胞淋巴瘤患者染色体畸变发生率为81.8%,明显高于1期以及Ⅱ期染色体畸变发生几率(71.4%),且差异有统计学意义(P〈0.05)。结论弥漫性大B细胞淋巴瘤患者染色体畸变率较高,且与恶性淋巴瘤临床分期密切相关。Objective To study the relationship between chromosomal abnormalities of diffuse large B cell lymphoma and its clinical staging. Methods Chromosome preparations were made by using modified method. Karyotypes were analyzed by stain of G-banding. The specimens were collected from peripheral blood and bone marrow of 25 patients with diffuse large B cell lymphoma. Results Metaphases that could be used for analysis were found in 22 cases of the specimens of peripheral blood. The achievement ratio was 88.0%. The detection rate of abnormal karyotypes reached 86.3%. The numerical chromosome aberrations included chromosomes l, 2, 3, 5, 7, 13, 14, 17, 18, 22 and X. Metaphases that could be used for analysis were found in 19 cases of the specimens of bone marrow. The achievement ratio was 76.0%. The detection rate of abnormal karyotypes reached 84.2%. Whereas it was 5% in the 25 specimens of control group, there was significant difference among this group and above-mentioned two groups ( P〈0.05 ) . The incidence of chromosome aberration in stage I /II and in stage N/IV was 71.4% and 81.8%. There was significant difference between different'stages ( P〈0.05 ) .Conclusion The abnormal karyotypes of diffuse large B cell lymphoma is high ( 80% ) . and its chromosomal abnormalities is significantly relative to clinical staging.
