持续静脉泵入奥曲肽治疗肠梗阻的临床疗效观察

摘　　要：[目的]评价奥曲肽在肠梗阻治疗中的临床价值和安全性.[方法]43例排除需急诊手术的肠梗阻患者随机分为奥曲肽组（22例）和对照组（21例）,两组患者年龄、性别、肠梗阻病因相比较差异无显著性（P〉0.05）,对照组按常规予禁食、胃肠减压、补液及纠正水电解质酸碱失衡治疗;奥曲肽组在上述常规治疗基础上联合醋酸奥曲肽25 μg/h,持续静脉泵入,连用3~10 d.观察两组患者症状及体征改善情况、胃肠减压引流量和腹部X线恢复时间、平均住院时间.[结果]两组治愈率比较差异无显著性（P〉0.05）,但奥曲肽组的腹痛腹胀缓解时间、胃管拔出时间、胃管引流量、腹部X线恢复时间,住院时间均优于对照组（P〈0.05）.[结论]奥曲肽治疗肠梗阻有效、安全,能显著缓解患者症状并缩短住院时间,值得临床推广.[Objective]To evaluate the clinical efficacy and safety of octreotide in the treatment of intestinal obstruction. [Methods] Forty-three patients with intestinal obstruction were randomly assigned to octreotide group（ n =22） and control group（ n = 21）. Patients who needed emergency surgery were excluded. There was no significant differences in age, sex and causes of disease between two groups（ P 〉0.05）. Control group was given routine therapy including abrosia, gastrointestinal decompression, fluid infusion and electrolyte rectification. Octreotide group received routine therapy in combination with continuous administration of octreoti- de 25μg/H by venous pump for 3-10d. The improvement of symptoms and signs, drainage volume after gastrointestinal decompression, abdominal X-ray recovery time and mean hospital day were observed. [Results] There was no significant difference in the cure rate between two groups（ P 〉0.05）. But the relief time of abdominal pain and distension, the extraction time and drainage volume of gastric tube, abdominal X-ray recovery time and hospital day in octreotide group were better than those in the control group（ P 〈0.05）. [Conclusion] Octreotide for the treatment of intestinal obstruction is effective and safe, and can markedly alleviate the symptoms and shorten hospital day, so it is worthy of clinical promotion.
