高强固位纤维防治牙折裂90例临床观察

摘　　要：目的:评价高强固位纤维修复牙体缺损的临床效果。方法:采用百强固位纤维带环状包裹粘结结扎技术对90颗牙体缺损患牙进行修复,其中牙合面洞型经根管治疗后30例(A组)、近(远)中邻牙合面洞型经根管治疗后30例(B组)、近远中邻牙合面洞型经根管治疗后30例(C组),修复,随访3、6、12个月,追踪修复效果。结果:90颗患牙3个月治疗有效率67.67%～83.33%,6个月治疗有效率76.67%～86.67%。12个月治疗有效率83.33%～93.33%。其中A组和B组各阶段有效率无显著性差别(P>0.05);C组有效率与A组和B组有效率有显著性差别(P<0.01)和(P<0.05);全部27例失败病例中不敢用患牙咀嚼占22例(24.44%),纤维带松脱5例(5.56%),未见基牙折裂、纤维带断裂和修复边缘继发龋发生。结论:高强固位纤维带环状包裹粘结结扎修复技术,对防治牙折裂疗效显著,缺损范围小者修复效果更显著,且具有对牙体组织破坏创伤少等优点,为临床提供了一种新型微创的固定修复方法。Aim： To evaluate the effects that teeth defects were restored with high-strength fiber.Methods： Ninety teeth defect were divided into three groups.Group A,30 cases which occlusal surface had completed root canal therapy.Groups B,30 cases which mesial surface or distal surface occlusal surface had completed root canal therapy.Groups C,30 cases which both mesial surface and distal surface occlusal surface had completed root canal therapy.All of the groups received the technique of high-strength fiber resin bond restoration,results were checked after three months,six months,and twelve months.Results： Among the 90 teeth,the efficiency of the three-month treatment was 67.67%-83.33%.When the six-month treatment was 76.67%-86.67%,and the twelve-month treatment was 83.33%-93.33%.The efficiency of each period in group A and group B did not have any statistically significant differences（P0.05）.But the efficiency in group C was significantly different from group A and group B obviously{（P0.01）and（P0.05）}.Among 27 failed cases,22 patients（24.44%） did not dare to use the sick teeth to chew,5 patients′（5.56%） fiber tape became loose.But the following situations did not occur： the abutment tooth was broken,the fiber tape were broken,or the margin secondary caries should be restored.Conclusion：The technique of bonding ligation with high-strength fiber tape is to consolidate and restore the teeth.It has a significant effect on preventing the odontoclasis for the patients whose teeth defects are smaller.Meanwhile,this technique has an advantage of doing less harm to the teeth.Therefore,it provides a new and fixed minimally invasive treatment for the patients.
